Overview
We are seeking to recruit a dedicated and experienced Adult Social Care Worker in the Complex Intervention Team - an integrated Mental Health service with North Somerset Council Adult Care and Avon and Wiltshire Mental Health Partnership Trust (AWP).
Responsibilities
* Work with older people with severe and enduring mental illness and dementia, providing a strengths based perspective within a multi-disciplinary setting.
* Undertake Care Act (2014) assessments and reviews.
* Work closely with social workers to carry out best interest decision-making, capacity assessments, risk assessments, and provide interventions to maximise independence and wellbeing.
* Collaborate with other professionals and agencies to ensure service users receive high-standard social care alongside their mental health care.
* Work within legislative frameworks including the Care Act (2014), the Mental Health Act (1983) and the Mental Capacity Act (2005).
* Undertake a range of social care and statutory tasks and provide social care knowledge within the integrated team.
* Provide timely reviews in line with Avon & Wiltshire Partnership and North Somerset organisational procedures and statutory duties.
* Maintain a clear framework for care plans and programmes of intervention for service users under Care Act and S117 in collaboration with service users and families/carers.
* Carry out Mental Capacity Assessments and Best Interest decision making with support of senior social workers.
* Ensure travel as required as part of role (valid driving licence and own transport).
* Adhere to safeguarding requirements and disclosure processes.
Qualifications and Experience
* Vocational qualification or experience at NVQ level 3 or 4 relevant to the service user group, or equivalent.
* Significant practical experience in social care and mental health.
* Sound assessment skills and ability to analyse eligible needs and think creatively to meet needs under the Care Act and S117.
* Ability to write clear, concise, high-quality reports.
* Ability to work unsupervised, with appropriate boundaries and to seek advice when needed.
* Knowledge and understanding of safeguarding protocols and thresholds for service users and carers.
* Ability to travel widely across North Somerset and out of area.
* Enhanced DBS disclosure certificate (relevant checks will be carried out before offer).
