Health Care Assistant/Support Worker

Role Overview
We are seeking compassionate and dedicated Support Workers join a small and consistent team providing support to adult client in their home with autism, ADHD, epilepsy and associated health conditions. You will be responsible to deliver stable, person-centred and high-quality support that promotes independence in a daily living, ensuring dignity, safety and well-being at all times. This role involves assisting clients with daily living activities, community activities, managing health needs, and developing skills for independence to live a happy and active life.
Key Responsibilities
* Provide consistent, structured daily support in line with the individual’s care plan and preferred routines.
* Use visual communication tools (whiteboards, picture cards, social stories) to enable understanding and reduce anxiety.
* Support with daily living tasks such as meal preparation, shopping, household routines and prompting with personal care and hygiene.
* Accompany and support participation in regular community activities.
* Monitor and support client’s health needs, and report changes to relevant healthcare professionals.Maintain accurate daily records, including health monitoring, activity logs, and incident reports.
* Work collaboratively with family, professionals and the wider support team.Participate in sleep-in shifts, ensuring safety and availability overnight.
* Assist with healthy and nutritious meal preparation, and manage weight.Ensure compliance with care plans, safeguarding policies, and health & safety regulations.
Requirements
* Experience in domiciliary care, community care or supported living and supporting clients with mental health, learning disabilities, autism, ADHD or behaviours that challenge.
* Knowledge of anticipatory medicines, symptom observation.A caring, patient, and empathetic nature and ability to build trust over time.
* Excellent communication and interpersonal skills with willingness to use visual aids and adapt communication style.Ability to work independently and as part of a team.
* A flexible approach to work, evenings, weekends, and sleep-in duties on a rota basis.
* Enhanced DBS Ability to remain calm in challenging situations and follow de-escalation strategies.
* Commitment to safeguarding, safety, and wellbeing of individuals.
Desirable:
* Full UK driving licence (desirable but not essential).
* Previous training in autism awareness, PBS, epilepsy awareness, or similar.
