An exciting opportunity has arisen to work within Swansea Integrated Therapy Team .Our service remit is centred around admission avoidance to both hospital and long-term care, supporting discharge from hospital through providing reablement within the person’s own home and complete assessments in relation to Disabled Facility Grant requests. This is an exciting opportunity to manager a complex case load of community individuals ranging from paediatrics to older adults.
Main duties of the job
To manage a defined caseload within specialist area, using evidence based /client centred principles to assess, plan, implement and evaluate the outcomes of intervention where appropriate; hospital, community, patients' homes, schools, outpatient settings and maintaining associated records. To organise this efficiently and effectively with regard to clinical priorities and use of time.
To ensure clinical records are kept to agreed standards and report accurately to others.
To work in partnership with clients, carers and all stake holders including the multidisciplinary/ multi agency team to identify relevant goals.
To provide leadership and supervision for junior staff, as delegated by supervisor /Team Leader.
To regularly supervise Occupational Therapy students on practice placements.
To participate in CPD and develop skills and knowledge through participation in developmental activities.

Essential criteria
1. Degree in Occupational Therapy (or other relevant OT qualification
2. Professional Registration with HCPC
Experience
Essential criteria
3. Experience of autonomous caseload management, comprising assessment, planning intervention and evaluation within relevant clinical area
4. Understanding of the Occupational Therapy process within specialist area of post
5. Knowledge of the principles of Clinical Governance
6. Broad knowledge of current best practice in Occupational Therapy with designated client group
Desirable criteria
7. Experience of research and/ or audit and implementing evidence-based practice
8. Previous experience of supervising staff
