Job overview We are excited to offer multiple Band 5 Registered Nurse opportunities across our Intermediate Care (IMC) areas at Warrington Hospital. These vacancies span a number of wards and services within the IMC footprint, offering flexibility and the opportunity to work in a variety of clinical environments. As a Band 5 Registered Nurse, you will play a key role in delivering high standards of care, working as part of a multidisciplinary team to support patients across their healthcare journey. The IMC Clinical Business Unit comprises a diverse range of wards, offering a broad and rewarding clinical experience: Acute medical wards: delivering care to patients with a wide range of conditions. Frailty and elderly care services: specialising in holistic care for older adults, including rapid assessment, dementia-friendly care, and complex discharge planning. Stroke and rehabilitation services: providing specialist stroke care alongside rehabilitation. Discharge Suite: facilitating safe, timely patient discharges, supporting patient flow and working closely with ward teams and community services. Together, these areas provide exposure to the full patient pathway which makes the IMC CBU an ideal setting for Band 5 nurses to develop clinical skills, build confidence, and progress their careers. Advert To be responsible for the unsupervised assessment of patient care needs and the development, implementation and evaluation of plans of care, ensuring high standards of evidence based care using a defined model of care. To undertake all nursing procedures in accordance with Trust policies and ensure that these procedures and policies are understood and adhered to by all staff, within own management responsibility. To communicate with patients and significant others with empathy and reassurance regarding, assessment, care and treatment. To maintain accurate and comprehensive nursing records using appropriate documentation. To facilitate early discharge planning enabling appropriate arrangements to be in place for patients leaving hospital, making referrals and providing information to community services, as required, in order to achieve a safe and timely discharge from hospital. Supervise junior staff, or unqualified staff, in all aspects of care. Ensure that staff has the knowledge, skills and resources to carry out care delegated to them. To work as part of a multi-disciplinary team to promote good communication and a multi-disciplinary approach to patient care. To ensure that the departmental co-ordinators are updated with clinical developments. Implement safe working practices as required by the Health & Safety at Work Act.
