Job summary
An opportunity has become available in the North Unplanned care service for a permanent Level 1 Social worker. We need a resilient and dynamic individual to join the experienced and enthusiastic team.
The work is varied and challenging and will focus on pathway 2 of the D2A service. You will be supporting and assessing people within a rehab focused bed based setting following an acute hospital stay. The core work will be undertaking statutory assessments in complex situations so knowledge of Mental Capacity and Continuing Health Care legislation is crucial. A knowledge of safeguarding adults will also be an advantage.
Main duties of the job
The successful applicant will be part of a multi-disciplinary team including nurses, physiotherapists, occupational therapists, discharge facilitators and support staff within the Haywood hospital and private residential and nursing homes. The team also works closely with external partners such as the Integrated Care board and Staffordshire County Council. The work is on a short term basis and cases are usually closed within 12 weeks; this means the successful individual will need to be able to work at pace and be able to move between competing priorities.
Development and progression
This is an ideal opportunity for any individual who is going to qualify this year or are newly qualified and wants to work within an unplanned care setting and access a variety of training via the Social Work Learning Academy (SWLA). There will be an expectation that the ASYE is completed within the first 2 years of employment and upon completion of the ASYE a my time meeting can be arranged to consider progression to band 6, if performing at that level.
