Community Staff Nurse

Dorset HealthCare University NHS Foundation Trust is currently offering a band 5 secondment for a 12‑month term in the Vale District Nursing team, based in Shaftesbury Community Hospital. The position is 30 hours per week and may require flexible working to meet service needs.
Excellent communication skills are required when caring for patients in their own homes. Any clinical skills required for this post will be supported.
We would welcome applications from newly qualified nurses and provide a comprehensive and fully supported Preceptorship Programme for new qualified healthcare professionals.
Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.
This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
Main duties of the job
Work as a registered nurse as and when required with community nursing teams, providing evidence‑based, high‑quality, holistic nursing care for patients in their own homes.
Carry out nursing assessments, treatment and care and liaise with other agencies as appropriate.
Be developed and supported by Dorset health care with training and development to support a registered nurse caring for patients in the community setting.
Assess, plan, deliver and evaluate nursing care for patients to meet their specific needs.
Assess the needs of patients in a variety of settings and formulate a plan of care. This may involve liaison with carers, other health professionals, GP’s, consultants, hospital nurses, social workers, hospice staff, specialist nurses, voluntary and statutory workers.
Provide evidence‑based nursing care to a defined patient caseload with the support of the District Nurse.

* Membership of the relevant Professional Body

* Supervised experience gained during clinical placements
* Demonstrable knowledge of assessment and therapeutic interventions in area of practice
* Ability to prioritise and organise workload effectively
* Recent previous experience within a comparable role
Managerial/Supervisory Experience
* Experience of demonstrating duties to junior staff
Information Technology/Resources
* Ability to evaluate data and produce reports using Microsoft Excel and Word
