An exciting opportunity has arisen for a Care Co-ordinator to join Surrey Heath Primary Care Network [PCN], supporting delivery of the new Network Contract Direct Enhanced Service [DES] specifications.
We are looking for a Care Co-ordinator to support our PCN, working closely with PCN Practices and the multidisciplinary team. Responsibilities include coordinating access to services, providing advice and information, and ensuring health and care planning is timely, efficient, and patient-centred.
The post holder will help patients engage with everyday activities designed to develop, maintain, or retrain skills for those with cognitive, physical, or mental health conditions. They will support continuity of care and act as a point of contact for families, residents, and professionals for patients in their caseload.
Key Responsibilities
1. Coordinate and navigate services for patients and carers across health and care sectors, working with social prescribers, health and wellbeing coaches, and primary care professionals.
2. Use population health data to proactively identify and support a cohort of patients with personalized care plans.
3. Assist PCN staff and patients in shared decision-making, utilizing decision aids and tools, and support patients' understanding of their health through the Patient Activation Measure.
4. Help patients manage their needs by answering queries, managing appointments, and providing information to support decision-making about their care.
5. Support the practice's Patient Participation Group [PPG].
6. Holistically integrate all aspects of a patient's care and support plan, focusing on what matters most to the individual.
7. Encourage and support patients to access training, employment, benefits, education, peer support, and personal health budgets where applicable.
8. Record key information accurately and comprehensively in clinical systems, adhering to data protection legislation.
