Position
Full and Part Time Social Worker positions in the Older People & Physical Disabilities Service across Lancashire. Salary £36,363-£40,777 per annum.
Location
North Lancashire.
Responsibilities
* Conduct strength‑based, outcome‑focused assessments and support planning.
* Support people in accessing local services, maintaining independence, promoting health and wellbeing.
* Collaborate with other agencies, professionals and informal carers to provide a holistic service.
* Participate in team discussions and support managers in service delivery.
Qualifications & Requirements
* Registered with Social Work England (SWE) or application in progress.
* Experience working with older people and/or individuals with physical disabilities.
* Strong empathy, communication and organisational skills.
* Car available for duties and car insurance for business use.
