Role Description
This is a part-time, on-site Social Worker role located in Dudley. The Social Worker will provide comprehensive support services to individuals and families, identifying their needs and coordinating appropriate care plans. Responsibilities will include conducting assessments, offering emotional and practical support, collaborating with healthcare professionals, and maintaining accurate records. The goal is to empower clients and enhance their quality of life while adhering to the highest standards of care.
Qualifications
Strong interpersonal, communication, and collaborative skills to build relationships with clients, families, and healthcare professionals.
Proficient in assessment, case management, and care planning to provide tailored support for a variety of needs.
Knowledge of local social services, healthcare systems, and community resources to connect clients with necessary support.
Empathy, patience, and advocacy skills to provide compassionate and client-centered care.
Bachelor's or Master's degree in Social Work or a related field, and relevant certification or licensing.
Ability to work effectively in a team and independently in an on-site setting.
Experience working within the healthcare or social care sector is highly desirable.
