Our rehabilitation unit is based at North Somerset Community Hospital. Elton Unit is an 8 bedded adult rehabilitation unit supporting the Pathway 2 acute discharge model.

Our patients present with a wide range of conditions, including stroke, orthopaedic, frail elderly, post-surgical cases, and long-term neurological conditions. We work closely with them to help achieve the highest level of personal independence prior to discharge.

Unit Nurse is responsible for the assessment, planning, implementation, and evaluation of care for patients, ensuring the highest standards of patient‑focused care. Patient safety, dignity, and respect are at the heart of everything we do.
Key Responsibilities
* Provide, monitor, and maintain a high standard of safe and effective nursing care through professional actions and clear verbal and written communication.
* With support from senior staff, undertake initial holistic assessments and develop, implement, and evaluate care programmes for patients who are acutely or chronically unwell, often with complex needs associated with frailty.
* Carry out a range of routine and non‑routine tasks, including:
o Medication administration
o Clinical observations
o Planning, implementing, and evaluating care plans
o Assisting patients with daily care needs
o Health promotion activities
o Venepuncture
o Planning and coordinating patient discharges
Work Pattern
Work across 7 days, including nights, weekends, and bank holidays, on a rostered basis:
* Day shift: 07:00 – 19:30
* Night shift: 19:00 – 07:30
