LeedsStudent Medical Practice and The Light Surgery Primary Care Network (LSMP &TLS PCN Ltd) are seeking to further expand their current Pharmacy team. This is an exciting opportunity for an enthusiastic and forward-thinking Clinical Pharmacist to join our multidisciplinary team, working alongside the PCN Senior Clinical Pharmacist,the well established Pharmacy team.

* This role required you to be fully on site and work across two practices (Leeds Student Medical Practice and The Light Surgery).
* This is a patient facing role, and there will be a mix of face to face and telephone appointments. Appointments will typically be 15 minutes.
* The role will also include completion of audits, assisting with triaging, and actioning tasks and letters.
* The population is comprised mainly of students and young professionals, so the ability to consult with this demographic is essential.
* There is a high prevalence of mental health within the population, so a significant portion of the role will be focused on mental health related Pharmacy interventions such as mental health reviews.
* This role does not come with designated parking, however there are other local parking options available.
Main duties of the job
The role offers significant involvement in direct patient care, with a focus on structured medication reviews and medicines optimisation. While experience in primary care and service development is desirable, it is not essential. The successful candidate will be supported to complete the CPPE Primary Care Pharmacy Education Pathway if not already undertaken, with dedicated mentoring from the Senior Clinical Pharmacist and GPs. Support will also be provided for those wishing to pursue Independent Prescribing (IP) qualifications following completion of CPPE Primary Care Pharmacy Education Pathway.
The post holder will:
* Work independently and collaboratively to deliver high-quality pharmaceutical care.
* Carry out complex polypharmacy medication reviews and structured medication reviews particularly for patients with long-term conditions.
* Support the PCN and surgery vaccination programmes
* Provide expert advice to patients and practice staff on medication and prescribing queries.
* Undertake medicines reconciliation and manage acute prescription requests.
* Contribute to the delivery of the PCN Directed Enhanced Service (DES), QOF, and local prescribing incentive schemes (GPOP).
* Promote the principles of medicines optimisation and contribute to audit and quality improvement initiatives.

Job responsibilities
Key Responsibilities:
* Provide clinical input into the safe and effective use of medicines within the PCN.
* Undertake structured medication reviews (Level 3) to optimise patient outcomes.
* Support the wider clinical team with prescribing queries and guidance.
* Identify unmet pharmaceutical care needs using risk stratification tools.
* Promote rational and evidence-based prescribing in line with NICE guidance.
* Participate in clinical audits, quality improvement initiatives, and national/local incentive schemes.
* Contribute to the training and development of other healthcare professionals.

Person Specification
Qualifications
* MPharm or equivalent pharmacy degree.
* Registration with the General Pharmaceutical Council (GPhC).
* Applicants must also be able to demonstrate relevant primary care experience during their pre-registration training year.
* Willingness to work with or experience working with young people and mental health related interventions.
* Completion of the CPPE Primary Care Pharmacy Education Pathway
Experience
* Collaborative working across the wider MDT, and working as part of a team.
* Strong knowledge of therapeutics and evidence-based practice.
* PCN Pharmacy experience.
* Experience completing audits.
Skills and Abilities
* Strong understanding of primary care prescribing.
* Awareness of the wider determinants of health.
* Excellent interpersonal, communication, and influencing skills.
* Capable of analysing complex clinical data.
* Ability to keep to time and manage your own workload.
* Ability to vaccinate adults in line with national immunisation programmes and local protocols.
* Ability to work across two sites.
* Familiarity with EMIS and/or SystmOne clinical systems.
* Ability to successfully triage and manage appropriate patients.
