Join our dedicated Community Nursing Team in Kingsbridge!

We are looking for a compassionate and motivated CommunityRegistered Nurse to deliver high-quality, patient-centred care to people intheir own homes. This is an excellent opportunity to work autonomously whilebeing supported by an experienced multidisciplinary team. Our core hours are0830-1700, covering a 7-day service.

The role requires positivity & excellent communication,leadership & caseload management abilities. A need to demonstrate integrity& initiative, sound clinical knowledge with evidenced based practice &the ability to support the team, approaching your work in a coordinated way

We work integrated with primary care, adult social care,therapists, urgent & intermediate care services alongside our specialistservices informing our ageing well MDTs which facilitates shaping our future todeliver great quality care for housebound individuals.

We strive to improve health outcomes & patientexperience. This role supports people in their own home and care homes with ourCare Home Service. Through expert advice & clinical support &treatment, promoting the optimum quality of life. Whilst providing an effective& efficient service delivery, utilising health promotion, wound, medication& continence management. Taking an active role with TEP, ACP with EOLPatients.


Main duties of the job

Community Registered Nurse - your role is vital in terms ofcoordinating peoples care through holistic evidence-based best practice inaccordance with National and Organisational approved polices/procedures.

You will be an autonomous practitioner and role model.Responsible for assessing, triaging, planning, implementation and evaluation ofcare, support the Community Sister/Charge Nurse in their absence.

Manage sensitive issues and overcome obstacles incommunication.

Required to undertake a robust induction, a mentor will beallocated to support your learning and development.

We offer training with required competencies for you tobuild on your knowledge, experience, and skills. You will be expected toundertake continued learning with accessibility to accredited courses.

Required to be competent, diagnose and investigate. Havesound knowledge and judgement providing evidence-based wound management andpressure ulcer prevention. Continence management, palliative, and end of lifecare.

Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.

Job responsibilities

MAIN DUTIES/RESPONSIBILITIES

5.1 Responsibility for People Management

To be responsible in the absence of the Clinical NurseSpecialist / Caseload Holder to manage own workload and delegate nursing careto appropriately skilled staff and ensure that these are supported.

To assume responsibility as delegated by a senior member ofstaff.

To act as a role model for all staff and studentsdemonstrating specialist clinical skills and high standards of practice andprofessional conduct.

To share knowledge and information to promote a cohesiveteam.

To take responsibility for your own personal andprofessional development, maintain competence, knowledge and skillscommensurate with the role.

To assess, teach, support, and supervise colleaguesincluding Pre-registration students and medical students.

To participate in team activities to develop and consolidatea cohesive and supportive team ensuring openness within the team.

To facilitate and participate in the development of teammembers, monitoring of sickness, line management, appraisals, mandatorytraining, competencies, clinical supervision and staff performance as directedby the Service Lead.

To participate in the recruitment and selection of newstaff.

To participate in coordinator/duty rota as required.

5.2 Responsibility for financial and/or physical resources

To exercise a duty of care in relation to the use of theorganisations equipment and resources in a cost-effective manner.

To use Livewell Southwest resources responsibly.

To work in collaboration with the budget holder, you may berequired to be an authorised signatory for ordering stores, equipment, andstationery.

To ensure that all loans of equipment are monitored andmaintained in good working order as per operational policy.

To monitor stock levels and ensure the supply levels aresufficient to meet the demands of the service.

To have a working knowledge of statutory and non-statutoryfunding and the legislation/guidance that underpins these funding streams,ensuring service users meet the eligibility criteria when advocating for/utilising services on the service users behalf (e.g. Care Act eligibility,Housing legislation etc)

5.3 Responsibility for administration

To maintain accurate records, which are confidential,contemporaneous, legible, and all care given to be documented. These recordsmay be paper or IT based system and must be maintained as specified in the LWSWPolicies, the relevant professional body guidelines and Government directives.

To oversee the standard of patients records by audit or peerreview and have a regular open discussion with team members.

To complete on time and submit all necessary forms,documentation, including IT data and forward as required by themanager/organisation.

To support in the triage process accepting appropriatereferrals directly from other disciplines, patients, carers and members of thepublic and/or refer to other agencies where appropriate.

To be responsible for organising own time management on adaily basis in line with caseload demands balancing patient needs with theadministrative aspect of the role.

To be involved in all aspects of clinical governanceincluding audits and research.

To preserve confidentiality and be aware of GDPR, Access toHealth Records and consent to treatment.

5.4 Responsibility for people who use our services

Be responsible for contributing to the holistic assessment,planning delivery and evaluation of care to patients within their own homes.Implement, monitor and maintain high standards of care delivery at all times.

To act as a patient advocate as required ensuring individualneeds, preferences and choices are delivered by the service.

Support CHC assessment and case management as part of theMDT for people at home where the community nurse is identified as the mostappropriate community team member.

To be involved in all steps of a patient's journey fromreferral to discharge.

To undertake health promotion as requested to all age groupsand contribute to planned health promotion activities e.g. NSFs, smokingcessation and long-term conditions. To act as a resource in providing adviceand support for patients, relatives, carers, and other agencies.

To have a flexible approach to the working day in order to meet the needs of the patient/service.

To support the monitoring of patient satisfaction andpromote appropriate service developments.

To signpost to appropriate services, statutory and voluntarybodies.

To have good communication skills which enable you toeffectively communicate with patients, their relatives and carers aboutsensitive and accurateinformation about your condition.

To support the prevention of hospital admissions.

To undertake comprehensive risk assessments of allsituations associated with the care of patients in order to ensure nurses andthe carers safety. This might include the assessment and provision of equipmentensuring it is used safely.

To provide holistic evidence based / best practice care topatients in accordance with National and LWSW approved policies/procedures andindividual care plans.

5.5 Responsibility for implementation of policy and/orservice developments

To be aware of and abide by Livewell Southwest approvedpolicies, patient group directives, standards and quality assuranceinitiatives.

To support development and review of policies, protocols,clinical guidelines, documentation systems and education materials to directown and others practice in line with best practice, professional forums,ensuring the clinical practice reflects national and local drivers.

To Preserve confidentiality and be aware of the GDPR, DataProtection Act, Access to Health Records and Consent for Treatment.

To report and record all incidents and near misses relatingto health, safety, security, fire, physical violence, aggression, and verbalabuse.

To monitor and maintain the health, safety and security ofself, others in the team and patients.

To integrate theory into practice by bringing new knowledgea from training into the practice environment.

Evaluate the impact of these training programmes, forpatients and carers, to ensure that they provide the necessary knowledge andskills to gain independence, safely manage changing circumstances and plan forunavoidable progression of conditions.

To act as a change agent, Organisation wide, initiating,facilitating, and supporting change initiatives, across professionalboundaries.

Monitor the emergence of new evidence and implement andevaluate research-based recommendations that are expected to improve care.

Identify and undertake areas for research and evaluationswithin own specialty, facilitating the involvement of appropriate staff.

5.6 Other Responsibilities

To work within the scope of the NMC professional code ofpractice.

Ability to use a computer, being responsible for timelyrecording of patient activities for IT, data collection, dealing with e-mailqueries, stock and equipment ordering.

To have an up-to-date personal development plan andprofessional portfolio.

To participate in an annual appraisal of their work in linewith the Knowledge and Skills Framework (KSF) where the job description will bereviewed and objectives set. In line with the annual development plan thepostholder will be expected to undertake any training or development requiredto fulfill their role.

To monitor own performance against agreed objectives throughpersonal development plans, NMC regulations and maintain professional expertiseby arranging and attending meetings, study days and in service training tosupport their own role and that of others.

To complete specific clinical competencies relevant to roleand ensure these are updated.

Problem-solve and liaise as soon as possible with theClinical Nurse / Caseload Holder issues that may arise.

Continually re-prioritising workload due to theunpredictability and demands for urgent visits.

6. COMMUNICATIONS AND RELATIONSHIPS

Links across & liaises with; primary care, secondarycare, mental health, voluntary & statutory organisations, SWAST, 111, Users& Carers, Members of the public, Integrated Localities, Specialist Servicesand Urgent and Intermediate Care (and all stakeholders as required).

To provide and receive complex, sensitive, and confidentialinformation and overcome potential barriers to communication, such as language,disability as well as dealing with concordance and barriers from patients tothe prescribed treatment.

To maintain and foster good relationships with professionalsand non-professional colleagues concerned with the provision or development of healthcare services.

Have good communication skills to effectively communicatewith patients, their relatives and carers about sensitive and accurateinformation about their condition.

Demonstrate highly developed communication skills requiredto take a lead role in case discussions/case conferences concerning serviceusers in their caseload.

To be aware of pressures facing your work colleagues andoffer support and ensure they are aware of LWSW support services available tothem.

To act as a clinical advisor on health promotions withintheir areas of expertise and act as a Link Nurse.

7. PHYSICAL DEMANDS OF THE JOB

You may be required to drive or travel for periods of timebetween patients homes in allweather / seasons and requiring lifting bags andequipment from the car boot on each visit.

Daily there is likelihood to exert moderate physical effortfor several long periods during a shift e.g. wound dressings and diagnostics inpatients homes, which require nurses to kneel, bending, sit on knees, bendingor lifting of limbs or buckets of water (up to 40 60 minutes per limb); thisinvolves manual handling and equipment.
