Job Summary
An integrated Occupational and Physiotherapy team in older peoples services (OPS) is looking for an enthusiastic and motivated individual. The role focuses on rehabilitation and reablement of older patients, including those with cognitive impairments such as dementia, supporting their safe and timely discharge from hospital to the community.
Responsibilities
* Manage and organise a designated caseload of patients and own professional activities across the older peoples wards.
* Provide specific skilled support to registered therapists in delivering technical therapy interventions to patients in a variety of settings.
* Use an evidence‑based, person‑centred approach in planning, implementing, evaluating and monitoring agreed interventions following guidelines and protocols to promote functional fitness of patients for safe discharge.
* Work flexibly over a 7‑day service rota.
* Communicate complex and sensitive information effectively to patients using skills of persuasion, motivation, explanation, negotiation and empathy, while addressing barriers such as hearing loss, speech difficulty, anxiety, fear and depression.
* Work with patients and carers to identify therapy goals for safe, timely discharge, using specialist, standardised assessment tools to assess skill deficits and functional needs.
* Collaborate with the patient to plan and implement a programme of interventions using treatment techniques that meet the patient’s needs for safe, timely discharge, applying professional, theoretical knowledge and clinical reasoning skills.
Qualifications & Experience
* Experience as a Therapy Assistant or a similar level role in a related field of health or social care.
* Key role supporting Occupational Therapists and Physiotherapists, with ability to work independently in assessment and safe discharge planning from hospital.
* Demonstrated experience delivering tailored rehabilitation and care for a wide variety of patients.
* Experience carrying out assessment for and provision of appropriate equipment.
* Experience working in an acute hospital setting is beneficial.

Working Hours
Monday to Friday; 5 days 0900 – 1530 (30‑minute lunch) or 4 days 0800 – 1600 (30‑minute lunch) with either Tuesday or Thursday as a non‑working day. Weekends on a rota basis (approximately 1 in every 5).
