About the role
The Cardiac Rehabilitation Service provides care, support, education and exercise options following a cardiac event such as myocardial infarction, percutaneous coronary intervention and cardiac surgery.
You will work alongside our multi disciplinary team to help provide this service. Ideally you will have some cardiac nursing experience, however, full training and support will be provided, so this is an excellent professional development opportunity.
We are looking for a caring, compassionate person, who has excellent communication skills and is able to work well autonomously and in a team.
This is a Trust-wide role, with bases at Normanton Health Centre and Dewsbury District Hospital. You will be expected to work across hospital and community sites as per service need.
The role is part-time, working 27 hours per week. Our service operates Monday to Friday, so in this role there is no requirement to work weekends or bank holidays.
Main duties
* Work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up to date records
* To carry out assessment and treatment of cardiac patients from inpatient stay through to full rehabilitation in the community with support and guidance from senior colleagues
* Provide specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required
* To support the team in the day to day running of the service
* To support and advise within the wider MDT on the delivery of the service

* Demonstrate innovation in practice
Experience
* Preceptorship/mentorship.
* Clinically competent to care for patients requiring cardiac rehabilitation
* Evidence of continued professional development
* Recent experience in a cardiology environment
