An exciting opportunity has arisen for a fixed term - 12 month Community Mental Health Nurse to join our Community Mental Health Team, based at the Bowes Lyon Unit, Lanchester Road Hospital, Durham. We are looking for a dynamic, positive and forward-thinking clinician with a passion for caring for people, their families and carers.

Main duties of the job

The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. The ability to work independently, effectively and collaboratively within Multidisciplinary Teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.

In the role you will be responsible for working with service users and their families. You will undertake a detailed history using a holistic approach and involve families and carers as part of our genuine commitment to co-creation. In this way you will promote positive experience and clinical outcomes from the beginning of the person's service journey.

There is a requirement to accurately record and document clinical discussions and assessment details on our clinical system and to ensure that all individual service user documentation is up to date and available to inform internal clinical meetings and aid consistent, safe and high quality decision making by members of the MDT and wider colleagues.

Qualifications

Essential

1. Current professional registration with Nursing & Midwifery Council RNMH or RNLD (RNSC - CAMHS only ) - RGN (If appropriate)
2. To be recognised practice assessor / mentor or work towards in an agreed timescale
3. Trained Clinical Supervisor (within 6 month probationary period)
4. Evidence of Continuing Professional Development

Experience

Essential

5. Working with people with mental ill health / learning disabilities
6. Working in a person centred therapeutic organisation
7. Working collaboratively with service users and their families/carers
8. Working in a multi-disciplinary team
9. Working in partnership with other agencies

Knowledge

Essential

10. Evidence based knowledge of best practice in caring for patients in the designated field.
11. Understanding of relevant legislation (. Mental Health Act, Mental Capacity Act)
12. Understanding of Safeguarding and its application in practice
13. Clinical Risk Assessment and Management and its application in practice
14. Clinical Governance and its application in practice
15. Confident working knowledge of clinical medicines management, including administration of injections
