Job Summary
We are looking for a Community Care Officer (Level 2) to join our Hospital Social Work Team. The role is based at Lincoln County Hospital and supports work across the Northern Lincolnshire and Goole NHS Foundation Trust hospital sites. It is a part‑time role of 13 hours per week, ideal for those seeking flexibility while making a meaningful impact. The successful candidate will assist people to leave the hospital safely and regain their independence.
Key Responsibilities
* Support patients and families to identify what matters most to them
* Complete strengths‑based assessments and reviews
* Arrange care and support that promotes independence
* Work alongside social workers, occupational therapists, nurses and ward teams
* Help patients navigate complex discharge planning
* Build strong relationships with health colleagues and community partners
* Contribute to safeguarding and risk management where needed
* Manage an own supervised caseload and play an important role in helping patients achieve positive outcomes
Qualifications
* Experience working in Adult Social Care, Health Care, Community Support, Hospital Discharge Services or Housing/Welfare Support
* Belief in strengths‑based practice
* Confidence working with people and families
* Ability to balance competing priorities
* Good communication and IT skills
* Enjoy working as part of a multidisciplinary team
* Willingness to make a difference every day
* Fluency duty: ability to converse at ease with customers and provide advice in accurate spoken English (per Part 7 of the Immigration Act 2016)
Preferred Experience
Experience of assessing needs, arranging support and working with partner agencies would be advantageous.

About the Team
Our Hospital Social Work Team supports patients across acute hospital settings, including Lincoln County Hospital and other NLAG hospital sites. We work closely with family, health colleagues and partner agencies to support safe discharges, promote independence and achieve positive outcomes for the people we serve.
