* Job Title: Support Worker

* Salary: £12.60 per hour
* Hours: Zero Hours, as and when required

* Driving: Full UK Manual Driving Licence
We are looking for a caring and enthusiastic individual with excellent people skills to join our Health and Care service. No professional experience? No problem. If you can make someone feel cared for and comfortable, you could be a perfect fit.
Our support workers provide practical and emotional support to individuals who may be struggling due to age, illness, or vulnerability. Your role will help enable people to lead healthier, more fulfilling lives. This career is not only rewarding but also offers extensive progression opportunities and the chance to build meaningful relationships with colleagues and service users.
Daily Responsibilities
* Providing practical and emotional support (telephone calls, welfare checks, shopping, etc.) and guidance to service users
* Supporting adults following hospital discharge to facilitate early discharge
* Responding to referrals from clinicians and health professionals
* Assessing needs and developing support plans for individuals
* Supporting service users post-discharge or to prevent readmission
* Transporting individuals to appointments or for errands such as shopping and prescriptions
Qualifications & Skills
* Full UK manual driving licence
* Knowledge of NHS and social care services
* Ability to improve service quality and handle queries professionally and confidentially
* Flexibility to work outside standard hours
