Overview
An exciting opportunity has arisen for a band 5 named nurse to work with patients undergoing renal replacement therapy at home. Previous experience in renal/haemodialysis is desirable but not essential, as full in-house training and support will be given. The team you will be joining is dedicated to training of both staff and patients.
Undertaking adequacy tests/training/refreshers to ensure patient safety at home undertaking their treatment.
Responsibilities
* Undertake the named nurse role for patients undergoing renal replacement therapy at home.
* Training patients on both Peritoneal dialysis (CAPD & APD) and haemodialysis (Nx Stage machine/5008s), ensuring a high standard of care is maintained throughout.
* Carrying out adequacy tests including PET/UKM for Peritoneal dialysis patients.
* Working as part of a team within the wider multi-disciplinary team.
* Undertake home visits regularly; being a car driver is essential.
* Actively contribute to setting and maintaining high standards of quality nursing care.
* Work collaboratively and co-operatively with others to meet the needs of patients and their families.
* Adhere to Trust policies, procedures and clinical guidelines and ensure compliance of others.
* Responsible for the effective and efficient use of clinical, physical and financial resources.
Qualifications and Requirements
* Excellent communication skills.
* Ability to work as part of a team or independently.
* A positive and proactive attitude.
* A desire to deliver a high standard of care to chronically ill patients.
* Flexible working will be considered.
* A valid driving license and ability to undertake regular home visits.
