Job Overview
The post holder will be responsible for ensuring the highest standard of clinical care is provided for patients referred to the team for District Nursing Services, which is delivered over a 24-hour period. In pursuing these duties, the post holder will ensure compliance with the NHS Constitution.
Main duties of the job
The NHS Plan sets a challenging agenda of improvement, expansion, and reform for health and social care providers. This includes devolution of power and resources to the front line to give health professionals who deliver care the freedom to innovate. The post holder will be responsible for ensuring the highest standard of clinical care for patients referred to the team for Palliative Care Service. The competencies described are based on the NatPaCT PCT Competency Framework for Nurses, Midwives, and Health Visitors (2003). The post holder will support the team leader in providing strong clinical leadership and ensuring the principles of clinical governance are implemented locally within the team. The post holder must adhere to the Code of Conduct published by the Nursing and Midwifery Council and work within the policies and guidelines of the PCT.

Person Specification
Knowledge
* Understanding of the NHS Constitution and its implications
* Registered Nurse
* Significant experience and competence in community nursing
* Able to assess patients with complex healthcare needs and develop care plans
* Palliative care experience
* Wound management including Doppler skills
* Phlebotomy experience/competency
Desirable Criteria
* Experience of risk management and governance
* Knowledge of working in a multicultural community
* Teaching & Assessment Qualification (e.g., ENB 998)
* Clinical supervision commitment
* Extended/Supplementary Nurse Prescriber
* Experience with PICC and Hickman Lines, IV Therapy, Cannulation, Ear syringing, Compression bandaging, Verification of death
