We are looking for a Band 6 Nurse to join our dedicated Substance Misuse Team in a development role within the speciality of Substance Misuse within a prison setting.

We directly deliver Mental Health, Physical Health, Pharmacy and Clinical Substance Misuse in an integrated healthcare delivery model to provide effective and responsive care to improve wellbeing and support better outcomes in the future.

Main duties of the job
As a registered nurse you will provide high-quality primary care services and a range of specialist interventions for offenders. In this role you will have a specialist interest in supporting those with identified substance misuse, dependency and related physical health issues in a Local Remand prison. At HMP Elmley, we deliver a nurse-led integrated primary care service and operate a specialist practice-based clinical model of care. You will be delivering high-quality clinical care within the Integrated Healthcare Service for the prison, demonstrating robust expert clinical skills and providing evidence-based nursing practice to those in prison. A clear competency framework and time scale will be agreed with your line manager to ensure your personal and professional development can be met in order to uplift your role to a Band 6 once your development plan and goals have been met. You will work closely with the Substance Misuse Clinical Lead and Practice Development Nurse to guide and support you on your development journey. We are able to provide preceptorship course as part of this development role if required.
Job responsibilities
* Referral management, screening assessment, triage, and evidence-based interventions
* Care planning and risk assessment
* One-to-one and group-work facilitation of a mixed and challenging caseload
* Pragmatic harm minimisation advice to patients
* Drug, alcohol and holistic health education to patients and colleagues
Leadership
* Provide supervision to junior staff and receive supervision as directed (line management and clinical)
* Develop and take on a specialist lead role within the Integrated Substance Misuse Service as appropriate
* Provide peer support and training in this area
* Contribute towards support, advice, consultation, and training for other prison departments
* Provide mentorship and leadership for junior staff
Clinical
* Complete comprehensive clinical assessment and develop immediate risk management and safety planning
* Instigate recovery planning for all prisoners arriving in HMP Bristol with a substance misuse need/dependency issue
* Identify and elevate safeguarding concerns at the point of arrival into HMPPS
* Assist in the clinical oversight of new arrivals over the first five days in prison
* Identify and respond to withdrawal or sedation indicators and request appropriate clinical interventions (e.g., Opiate Replacement Therapy, alcohol detoxification, benzodiazepine detoxification)
* Provide harm minimisation and health education, including Naloxone administration
* Deliver evidence-based specialist care for alcohol and substance dependency, and signpost for co‑morbid conditions
* Ensure effective and timely patient care and equivalence to community standards
* Coordinate joint and collaborative care planning for patients with additional complexities including release planning
* Assess learners with learning disability and provide appropriate interventions
* Participate in resource centre services under clinical lead direction
* Work psychologically minded with prisoners to achieve agreed goals and health outcomes
* Participate in multidisciplinary team reviews; deliver joint reviews with patients' Psychosocial Recovery Workers, ISMS prescriber
* Conduct training on screening, health promotion and prevention, including vaccinations
* Maintain proactive, evidence-based interventions, health promotion/prevention activity, and management of acute and long-term conditions using a stepped-care approach
* Ensure individualised care plans are regularly reviewed with MDT meetings tailored to patient needs
* Engage in ongoing training in evidence-based drug and alcohol treatment approaches
* Help patients manage health at all stages, from oral health to long-term conditions
* Embed health promotion into all service aspects
* Maintain comprehensive and timely electronic clinical records and confidentiality
* Participate in annual appraisal reviews and demonstrate reflective practice via Knowledge and Skills Framework
Research
* Carry out administrative tasks including research, audit, data and activity statistics
Communication
* Collaborate closely with community drug and alcohol services and other services to ensure appropriate sharing of information, transfer and continuity of care for all prisoners
* Provide comprehensive and timely reports and expert substance use/treatment information to MAPPA, police, probation services, solicitors and court liaison teams
* Maintain close working relationships with all prison staff and partners to improve patient care outcomes, including participation in sentence planning, resettlement, safer custody and ACCT processes
* Communicate effectively in complex situations and adhere to relevant health and safety policies
* Resolve inter‑agency issues that may arise across organisational boundaries
Custodial Responsibilities
* Assume personal responsibility for the security of issued keys
* Understand and comply with prison orders, procedures and instructions
* Comply with all security requirements
* Respond to any situation indicating a threat to security or safety and complete appropriate incident, security and injury reports
* Report breaches of discipline and record untoward incidents according to local protocol
* Contribute to effective risk assessment and management procedures

Salary
£41,959 to £50,117 a year per annum.
