Are you anexperienced, Advanced Clinical Practitioner who is looking for part time hours? If so we would like to hear from you.

We have a new role to recruit an ACP for 15hours per week over 2 days.

Sina Health Centre is a thriving, friendly NHS GP practice located in Willenhall witharound 7000 patients and a Good overall rating from CQC.

To add toour existing comprehensive range of staff, we are recruiting an ACP to join ourteam, we are committed to providing high quality care to our patients whilstcaring for the well- being of our staff.

The idealcandidate will take responsibility as a senior member of our dedicated nursingteam, working closely with our management team, GPs, nursing team and administrationteam to provide patient -centered care.

Main duties of the job

You willneed to work collaboratively with the team to meet the needs of the patientswhilst demonstrating safe clinical decision making and offering excellent careto our patients.

You will beexpected to focus on delivering evidence based practice to patients with longterm conditions and acute illnesses.

As anautonomous practitioner, you will be responsible for the care delivered andwill need to demonstrate critical thinking and clinical decision making skills to deliver expert care, including assessment and diagnosis skills for patients within the practice

You willconduct both face to face and telephone appointments and workdynamically and collaboratively to meet the needs of our patients on a dailybasis.

In additionto these responsibilities you will be responsible for adhering to policies andprocedures

We activelyencourage professional development and training to help you explore yourpersonal development and support the development of the practice.


About us

We are a friendly, patient focused GP practice serving a patient population of around 7000 patients. Our team has a wide range of skill mixed administrative and clinical team that is complimented by ARRS staff provided through our PCN. We pride ourselves on delivering high quality care in a supportive and collaborative environment.

Our practice is well maintained with free adequate parking to staff and patients; it also has strong links to local health services including a pharmacy which is located on the same site.

CQC rated good.


Job responsibilities

The postholder will autonomously undertake assessment and treatment of adults (andchildren if qualified to do so ) who present with an illness

To ensure ahigh quality patient centered care

Complete annual reviews discussing treatment options andlifestyle modification for patients with chronic disease

Be competent in interpreting results of investigationsordered e.g. blood tests

Prescribe medication for patients as needed in line with thelocal prescribing guidelines


Person Specification


Experience

* Experience in General Practice/ Walk-in Centre/ Urgent Care Centre/ Emergency Department/Out of Hours Service desirable
* Evidence of undertaking clinical audits
* Up to date knowledge of DOH and NICE guidelines
* Knowledge of use of IT/library searches knowledge
* Can discuss in depth current clinical and managerial issues regarding safeguarding adults/children and domestic abuse
* Evidence of teaching and mentoring junior staff


Qualifications

* Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council
* Advanced Nurse Practitioner (RCN Accredited) Or Advanced Clinical Practitioner (RCEM Accredited) or equivalent qualification.
* Experience in General Practice/ Walk-in Centre/ Urgent Care Centre/ Emergency Department/Out of Hours Service desirable
* Up to date knowledge of DOH and NICE guidelines
* Knowledge of use of IT/library searches knowledge
* Can discuss in depth current clinical and managerial issues regarding safeguarding adults/children and domestic abuse
* Experience in a Primary Care setting
* It is preferred that the ideal candidates is a level 7 (MSc) Practitioner, although candidates with accreditation or a MSc course to ACP status may be considered.
