Overview
Greater Manchester Mental Health NHS Foundation Trust is advertising for two full‑time Patient Flow and Capacity Practitioners to support the Adult Forensic Services (AFS) referral and assessment pathway, including repatriation and out‑of‑area placements (OAPs). The post requires direct patient contact and expertise in forensic assessment, discharge planning and clinical recommendations.
Reporting
The successful candidates will report to the Operational Manager for Forensic Community Services and work closely with the Service Manager and Lead Consultants.
Responsibilities
* Provide management and clinical leadership for the AFS pathway, including the Admission and Referral Panel.
* Monitor and triage the Single Point of Access (SPOA) for forensic service referrals.
* Oversee bed capacity, patient flow, timely admissions, out‑of‑area placements and waiting lists.
* Collaborate with AFS, Local Provider Collaborative (LPC) and other agencies to identify appropriate admissions and maintain service users in the least restrictive environment.
* Work with case managers to monitor pathways of patients in non‑GMMH commissioned forensic services.
* Triage referrals to national service specifications and coordinate assessment completion.
* Manage and prioritise patients on the waiting list based on clinical presentation.
* Establish care pathway and core interventions for patients admitted to AFS.
* Identify discharge pathways at admission, ensuring length of stay matches need and risk.
* Ensure assessments and outcomes are completed within national guidelines and timescales, including outcome letters.
* Maintain the AFS referrals tracker and update alongside administrative colleagues.
* Communicate decisions with referring teams and document outcomes on the clinical database.
* Provide oversight of reporting to the LPC regarding admissions, discharges and referral activity.
* Advise on flow and process design to support seamless patient journeys, integrating national and local policy.
* Develop and implement guidance, plans, processes and policies, applying project management where appropriate.
* Ensure systems are in place for key performance indicators and data capture.
* Assist in operational and contingency planning for specific events with SLT and Head of Emergency Planning.
* Conduct complex assessments for patients requiring secure care or currently in secure care; make recommendations for multidisciplinary teams.
* Attend professional meetings, inpatient reviews and clinical team meetings.
* Lead and chair weekly patient flow meetings and oversee assessments.
* Participate in the on‑call rota.
Requirements
* Clinical experience in forensic assessment and patient flow.
* Ability to work collaboratively across agencies and partners in Greater Manchester.
* Strong communication and leadership skills.
* Capacity to travel long distances; flexible working arrangements will be considered.
* Working knowledge of national guidelines and timescales for forensic services.
* Experience with data capture and reporting to partner agencies.
