An exciting job role has recently become available at the Stockport Metropolitan. They are looking for a new member for their Adult/Hospital Discharge Team. What will your responsibilities be? In this role, you'll collaborate with Pennine Care colleagues to enhance mental health services and outcomes for individuals transitioning out of the hospital. Your responsibilities will include ensuring safe and timely discharges for those with functional or organic mental health conditions, working closely with individuals, carers, health professionals, advocates, and third-sector colleagues. You'll co-produce Care Act assessments, support plans, and risk assessments, conduct Mental Capacity Assessments, and chair Best Interests Meetings. Additionally, you'll contribute to improving the discharge process through effective teamwork and partnership.
