The Carers Intensive Support Team are looking for a hard working, dedicated and compassionate individual to join us in supporting the health and wellbeing of those caring for a person with suspected or confirmed dementia, frailty, or an older person with mental health concerns.
You would be joining a unique team dedicated to supporting family carers responding to crisis and to look after their physical, mental, emotional and social health to enable them to maintain their caring role and build resilience for the future.
You will need to be willing to work across Thurrock Council Area, to attend Carers Groups and Events as well as cover annual leave and sickness absence.
You will need a valid Full UK Driving Licence and access to a car as the role requires travel across South Essex.
Main duties of the job
Working with Dementia & Older People’s Mental Health Team, Frailty Team and Community partners across Thurrock to identify and support carers whose physical and mental health is deteriorating due to deconditioning and the strain of caring. (Carers of any age caring for a person with suspected or confirmed dementia or a carer 65 and over caring for someone who has mental health concerns or who is frail aged 65 or over in the most complex caring situations with time limited interventions to achieve their optimal goals)
Based in Thurrock OPMH Team and working with Frail + partners using FrEDA (Frailty, End of Life & Dementia Assessment) and the Carers Health & Wellbeing Check a personalised care and support planning tool linking the PCNs, Community teams and Acute partners through S1.
You would identify and support opportunities for Carers Health Checks to
* Ensure early identification of respiratory concerns.
* Ensure carers uptake of flu, pneumonia and other vaccinations to prevent hospital admissions and ability to maintain caring role.
* Ensure opportunities to discuss lifestyle choices such as smoking, diet, inactivity, that may contribute to increased risk of cancer.
* Undertake or refer for BP, Pulse and other physiological monitoring.
* Early identification of Atrial Fibrillation and lifestyle advice to ensure early identification of hypertension concerns and education.
