Luther Street Medical Centre is a nationally recognised GP service providing high‑quality healthcare to people experiencing homelessness across Oxfordshire. We’re expanding our reach and we need a dynamic, compassionate Band 5 Outreach Practice Nurse to help us deliver excellent care directly to the communities who need it most.

It’s a chance to take your skills out into the community, hostels, day centres, temporary accommodation, and street settings bringing healthcare to people who cannot easily access it.

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

Interviews will be held week commencing 6th July 2026.


Main duties of the job

Run nurse led clinics acting independently but seeking advice and support from the Band 6 Nurses, Nurse Lead and GP team as required, at Luther Street Medical Centre (LSMC) or other outreach settings e.g. hostels.

Undertake assessment and treatment of patients accessing the service in a professional and safe manner, in accordance with national and local guidelines and protocols on clinical care.

Provide clinical assessment, nursing diagnosis and treatment of patients in the practice, the community and in the hostels and shelters that work with homeless people. Ensure timely escalation to appropriate members of the multi‑professional team where this is required.

Detailed job description and main responsibilities

Work effectively as a committed member of the wider multidisciplinary team at LSMC to ensure continuous service delivery.

The post holder is expected to actively participate in the development of service provision, working flexibly to support and meet the changing needs of the service.

Provide and maintain nursing care to the optimum standard possible within the resources provided.

Work with interpreters to ensure equality of care to all patients, being aware of cultural issues and barriers. Be sensitive to needs of vulnerable clients e.g. asylum seekers, refugees, the rough sleepers and hostel dwellers.

Liaise with other statutory services as well as voluntary organisations in the interest of the patient within the boundaries of client confidentiality and seek guidance when unsure.

Assist the multidisciplinary team with planning any outreach sessions as requested by the Nurse Lead and working collaboratively with other relevant services.

Undertake such tasks as are appropriate to own experience and nursing expertise as requested by senior nursing staff.
