Overview
Support clients to work towards and achieve their personal therapy rehabilitation goals. Works within the multidisciplinary team in a primary care setting including clients’ homes, residential homes and clinics. Assists and carries out delegated duties in line with provision of quality evidence-based practice to clients, following agreed protocols to implement and progress therapeutic care plans and chronic disease management reviews with the aligned caseload.
To work a variety of shift patterns which includes weekends and bank holidays on a rota basis. Core working hours are 8-4pm and 1-9pm, 7 days a week. A driver’s license and access to a car is essential.
Responsibilities
* Responsible for implementing and supporting rehab plans written by Physiotherapists, Occupational Therapists and Nurses.
* Involve clients and carers in the planning of care/rehabilitation programmes and encourage self-management where appropriate.
* Contribute to client goal planning sessions and show initiative in areas where competent and as agreed by senior staff.
* Show good understanding of communicating in difficult circumstances with empathy and sensitivity in an understanding and factual way.
* Form professional relationships with clients and communicate condition related information to them in a way that respects their views, autonomy and culture.
* Monitor client progress, giving prompt feedback to family, carers and the multidisciplinary team.
* Manage own delegated caseload, duties and travel throughout Bedfordshire, reprioritising as necessary.
* Support and work on behalf of different health professionals within the Integrated Community Services.
* Undertake training and work within agreed competencies providing therapy-related care to clients delegated by registered nurses as required, including:
o Basic wound care management
o Catheter management
o Bowel and stoma care management
o Eye care
o Blood glucose monitoring
o Clinical observations
o Medicines support
1. Assist and perform relevant diagnostic procedures as requested by appropriate clinicians.
2. Identify relevant basic equipment and feedback to case holder with appropriate clinical reasoning.
3. Deliver, fit and demonstrate basic equipment to client and appropriate others, ensuring client safety.
4. Assist with or administer as required prescribed medication according to Trust policy.
Qualifications and Requirements
* A driver’s license and access to a car is essential.
* Willingness to travel and work across Bedfordshire as required.
* Training within agreed competencies providing therapy-related care as delegated by registered nurses or other health professionals within Integrated Community Services.
* Experience or training in the listed care areas as part of the role (e.g., basic wound care, catheter management, bowel and stoma care, eye care, glucose monitoring, clinical observations, medicines support).
