Job Summary
NTGP has a fantastic opportunity for a Practice Nurse to join our multi-disciplinary team. The position will require sessions from both locations (on different days). We are flexible with sessions available. The days can be negotiated to suit you, the team & patients.
The Practice Nurse will act autonomously within their professional scope of practice, providing care for patients face-to-face, via telephone consultations, and home visits.
You will be responsible for providing medical services to our patient population, managing a caseload dealing with a wide range of health needs. You will be supported by, and support, the broad clinical and non-clinical team within the practice.

About Us
We are a large, forward-thinking practice serving 15,000 patients, providing quality healthcare and outcomes in the North Trafford area. Our locations are Chester Road and Seymour Grove.
The team at NTGP includes 3 GP Partners, 8 Salaried GPs, 2 Physician Associates, 2 Pharmacists, and 6 Nursing staff, along with a non-clinical team of over 30 staff managed by the Business Manager.
Our practices are rated as Good by CQC. We pride ourselves on delivering quality care and fostering a strong training and development ethos. We are part of the North Trafford Primary Care Network, with one partner serving as Clinical Director.
We use EMIS Web and DOCMAN as our clinical systems, along with Ask My GP for digital patient engagement.

Job Description
Job responsibilities
* Practice independently in accordance with NMC code of conduct and practice guidelines.
* Maintain accurate documentation of care and utilise IT effectively.
* Ensure ongoing personal and professional development through clinical supervision networks.
* Provide expertise within the practice, advising nursing staff as appropriate.
* Administer adult and child immunisations and vaccinations per national and local programmes. Be competent in anaphylaxis and resuscitation techniques. Ensure safe vaccine storage, rotation, disposal, and maintain stock levels.
* Lead in areas such as Asthma and COPD management, establishing and running disease management clinics, and providing specialist nursing care.
* Manage chronic diseases including asthma, hypertension, COPD, CVA/TIA, CHD, and conduct risk assessments and management. Initiate diagnosis processes for suspected chronic diseases.
* Maintain disease registers.
* Initiate and carry out health promotion and screening programmes, including cervical and sexual health screenings.
* Coordinate efforts as Nurse Lead for Infection Control.
