About the team
You will join an energetic and supportive team striving to support people in Southampton to live active, safe, and independent lives. The City Wellbeing Team supports adults living in the community with physical disabilities or dementia diagnoses, delivering statutory services to individuals, families, and carers, either in their homes or in residential/nursing placements.
The social work roles include undertaking reviews under the Care Act 2014, Mental Capacity Act 2005 assessments, contributing to multi-agency planning, providing interventions, negotiating and coordinating resources, and conducting s.42 safeguarding enquiries.

Salary: The role's salary band is £37,280 - £40,778, with a starting salary of £37,280 and annual progression within the band.
