What you will do
* Collaborate with Reablement Officers, social workers, and other health professionals to tackle Reablement cases requiring Occupational Therapy input.
* Conduct comprehensive Occupational Therapy assessments, including Double Handed Care reviews, assisting with moving risk assessments, and planning for straightforward adaptations such as wet rooms and stairlifts.
* Utilise a strengths‑based, person-centred approach to empower residents to enhance their independence and wellbeing.
* Prescribe and promote the use of equipment and adaptations to improve quality of life, aiming to reduce the need for ongoing formal care.
* Guide and support Reablement Officers and social workers by helping them work towards SMART goals, undertaking joint visits, and demonstrating effective techniques.
* Participate actively in team meetings, contributing to capacity management across Reablement Teams.
* Maintain clear and accurate records using Social Care electronic systems.
* Provide professional advice and support to staff across Adult Social Care and other key stakeholders.
Qualifications & Requirements
* HCPC registration is required.
* Driving requirement: Ideally a car driver.
