Role Overview
The post holder, taking the lead for the Healthy Families Team, will be responsible for the case management, assessment, triaging, planning and delivery of care for parents, children and young people using evidence based interventions. This will be delivered in line with national and local guidelines and public health priorities.
They will provide an operational overview of the Healthy Families Team allocated to a defined geographical population. The team will work proactively using a partnership approach to promote health and contribute to tackling health inequalities at a level of competency required for the post.
They will work with children, young people and families within their home environment, school or other identified community setting. A key aspect of the role is to work flexibly to meet the needs of the service as required.
Key Responsibilities
* Case management, assessment, triaging, planning and delivery of care for parents, children and young people using evidence based interventions.
* Operate the Healthy Families Team for a defined geographical population.
* Use a proactive partnership approach to promote health and tackle health inequalities.
* Work with families in home, school and community settings.
* Provide services to antenatal women, complete the Healthy Child Programme for under 5s.
* Deliver enhanced programmes of care within school and community settings for the 5‑19 age group.
* Provide breastfeeding support.
* Lead the skill mix 0‑19 cluster team.
Team Overview
The Ashfield Healthy Families Team provides a quality service to the local population and works in partnership with local Family Hubs. It is a skill mixed team led by Specialist Public Health Practitioners, delivering high standard of care with staff receiving good training and induction within a friendly supportive environment.
Qualifications
* Full UK driving licence and vehicle for business use.
* DBS check required; candidates will be required to pay for their DBS check (£26.40 standard or £54.40 enhanced), cost waived after two months of employment.
* Ability to make reasonable adjustments for disabled individuals in line with the Equality Act 2010.
* Experience in public health, case management and partnership working is desirable.
