Clinical Pharmacist

This is your opportunity to join our clinical pharmacy teamsworking across one of the largest acute care providers in the North East withpositions available at both University Hospital North Durham and DarlingtonMemorial Hospital. You will also gainspecialist clinical pharmacy experience in mental health through a rotationalplacement at Tees, Esk and Wear Valley NHS Foundation Trust.

All of our juniorpharmacists are enrolled on the RPS Foundation pharmacist pathway and,supported by our dedicated junior pharmacist mentors, you will provideward-based pharmacy services whilst ensuring that you progress against the fourpillars of the framework. Formaltraininng is provided through regular in-house teaching sessions and enrollingonto a choice of postgraduate diploma in a timeframe of your choice.

We are proud that the majority of ourpharmacists are independent prescribers and therefore we will be able tosupport you with the IP qualification at the time point that it is right foryou.


Main duties of the job

All of our Pharmacists spend more than 90% of their time onMedicines Optimisation activities with minimal dispensary commitments. Ourpharmacy technicians undertake much of our medicines reconciliation activitieson our wards, allowing pharmacists to focus on excellent patient-focusedclinical pharmacy.

As a department, we pride ourselves on thequality of the pharmaceutical care we provide for patients. We work at the leading edge of clinicalpharmacy practice with our teams winning national recognition for innovative high-quality patientcare. The successful candidate(s) willbe expected to deliver a high standard of near patient specialistpharmaceutical care to patients. Theywill support the pharmacy team to deliver governance, and service improvementto their specialist area and will help shape the strategic direction ofpharmacy by supporting the development of innovative new services. Experience will also be gained in cancerservices and the dispensary.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities

Participating in the structured rotational programme, toinclude aseptic and community servicesacross the sites as necessary

Participatingin the Foundation Level competency framework for Band 6 pharmacists

Providingmedicines management, medicines optimisation and medicines reconciliationservices to help achieve department priorities and targets on a designated wardor group of wards as decided by the Lead Pharmacist.

Helpingintegrate role of medicines reconciliation into the existing medicinesmanagement framework.

Helpingimplement local policies and guidelines on good prescribing practice.

Influencinglocal practice to ensure safe medication use, and to ensure that their ownpractice and the practice of others effectively acknowledges good prescribingpractice, patient safety and risk minimisation.

Being apoint of contact for nursing and clinical staff regarding medicine managementissues.

Providingmedicines information to healthcare professionals utilising the range ofinformation resources available, including the library and the internet, and tomake an informed decision on the appropriate response required.

Beinginvolved in providing clinical pharmacy service to designated multidisciplinaryteam structures e.g. out patient clinics, pre assessment.

Providing highquality patient educationon medicines usein both in-patientand outpatient settings.

Engagingin activities relating to clinical effectiveness including: drug use review,clinical audit and clinical self-audit and to contribute to ongoing measures ofservice evaluation.

Acting as a professional checker both in the dispensary and/ or aseptic service.

Taking part in the on-call rota


Person Specification


Statutory Registration

* Registered as a trainee pharmacist or practising Pharmacist with the GPhC


Experience

* Sufficient experience to provide near-patient clinical pharmacy and dispensary services
* Experience of developing good multi-disciplinary team working with other health care professionals
* Experience of Hospital Pharmacy


Special Requirements

* Good oral and written communication skills Good interpersonal skills
* Able to build good relationships with colleagues Team player
* Desire to develop clinical skills
* Able to prioritise own workload
* Self-motivation and ability to motivate others
* Evidence of presentations and/or teaching


Qualifications

* Masters Degree in Pharmacy or equivalent Evidence of CPD and education


Special Skills & Knowledge

* Good understanding of current pharmacy/ NHS issues Knowledge/experience of clinical pharmacy activities Ability to apply theoretical knowledge to ward based situations
* Evidence of patient facing clinical pharmacy activities
* Ability to answer complex queries through a sound knowledge of MI resources
* Supervision of others
