We are very excited to be recruiting a nurse associate for the Burnley West PCN. A knowledge of PCNs and what we are trying to achieve is a must in this role.

We are looking for someone who is passionate about the role of the nursing associate and the opportunity work with a multi-disciplinary team to deliver patient care. We are looking for someone who is flexible, self-motivated, willing to embrace change and be proactive in continuously improving service delivery.

Please note you could be driving to other practices/locations within the Burnley West PCN area.


Main duties of the job

The Nurse Associate will:

- Be responsible for delivering evidence based, high quality,patient centred, compassionate care every time.

- Be an accountable professional

- Work as part of a designated clinicaland care team delivering care that focuseson the direct needs of the individual

- Have the ability to work without directsupervision at times

- Have the capacity to deliver care independently in line with theindividuals defined plan of care within theparameters of practice of the Nurse Associate role

- Be able to access clinical and care advicewhen needed

The responsibility of the postholder is to participate in:

- The planning and delivery of Enhanced Health in Care Homes model ofproactive care that is centred on the needs of individual residents, theirfamilies and care home staff

- Chronic Disease Reviews with housebound patients

- Clinical Sessions within a practice setting

Job responsibilities

This is a list of general duties, please refer to the attached documentation for a full description of the role

- Work as part of the PCNs MDT to provide andmonitor care, under direct or indirect supervision

- Improve safety and quality of care at everyopportunity

- Contribute to the delivery of integrated care

- Work with the PCN MDT to ensure delivery ofnursing associate duties complement existing workforce

- Provide support and supervision to trainingnursing associates, healthcare assistants, apprentices, and those on learningassignments/placements as required

- Support registered nurses to enable them to beable to focus on the more complex clinical care

- Develop relationships across the MDT to supportintegration of the role across health and social care including primary care,secondary care, and mental health

- Perform and record clinical observations such asblood pressure, temperature, respirations, and pulse

- After undertaking additional training, provideflu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks asrequired by the PCN, in line with the competencies of the role

- Promote health and well-being to all patients,for example undertaking the NHS health check

- Care for individuals with dementia, mentalhealth conditions, and learning disabilities

- Advise patients on general healthcare andpromote self-management where appropriate, including signposting patients topersonalised care colleagues and local community and voluntary sector services

- Communicate proactively and effectively with allMDT colleagues across the PCN, attending and contributing to meetings asrequired

- Maintain accurate and contemporaneous patienthealth records

- Enhance own performance through continuousprofessional development, imparting own knowledge and behaviours to meet theneeds of the service.

- Maintain a professional portfolio, identifyingrole and skill developments in linewith the NMC Code of professional practice, ensuring NMC revalidation requirements are met

- Keep up to date with current nationaldevelopments in nursing practice and policy

- To contribute to PCNwide/departmental policies, procedures, and guidelines

- Deliver high quality, compassionate care underthe direction of a Community Link Nurse(or other registered care professional dependent on setting) with afocus on promoting health and independence.

- Have proficient attitudesand behaviours compatible with ELA & NHS Values

- Carry out specificdelegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of lead nurse or other registered care professionalsdependent on setting

Knowledge, training,and experience

- Work within clearly defined accountabilityframeworks and boundaries of limitation to provide the best possiblecare in partnership withcolleagues, patients, and their carers

- Contribute towards developing a culture of learning and innovation, developing high quality learning environments

- Communicate effectively across a wide range ofindividuals, the public, and health and social care professionals

- Communicate information received from theindividuals, carers and members of the team appropriately

- Ensure all patientrelated information is treated appropriately, always adhering to theprincipals of confidentiality

- Exercise personal responsibility and workindependently within defined parameters of practice, takingthe initiative in a varietyof situations and performing a range of clinical and care skills consistent with the role, responsibilities, and professional valuesof a Nursing Associate

- Exercise judgment in assessing patientcondition, comfort and wellbeing using analysis of a rangeof possible factors

- Maintain a professional appearance in line withthe dress code.

Clinical responsibilities

(The followinglist is indicative as tasks and responsibilities will vary depending on the care settingthe Nurse Associateis working in.)

The Nurse Associate will:

- Provide and monitorcare.

- Develop Understanding of all elements of thenursing process and be able to assist the lead nurse in the on-goingassessment, planning, management, and evaluation of care

- Support individuals with all aspects of careincluding daily living, providing person-centred care and promotinghealth and independence through awareness raising andcare navigation to other professionals as appropriate

- Perform and record clinical observationsincluding blood pressure, temperature,respirations, pulse, and NEWS

- Accurately record nutritional and fluid intake

- Ensure the privacy,dignity and safetyof individuals is alwaysmaintained

- Demonstrate the ability to recognise changingpriorities seeking advice andguidance from the Registered Nurse or other registered care professionals as appropriate

- Report back and share information with the registered nurses on thecondition, behaviour, activity, and responses of individuals

- Recognise issues relating to safeguarding adults and report any problems or raise concernsto the appropriate registered care professionals

- Assist in the assessment of and contribute tothe management of risk across severalareas within the environment where care is beingadministered

- Assist in the implementation of appropriate action to meetthe specific physical, emotional, and psychological, social, cultural,and spiritual needs of individuals and carers

- Assist in the delivery of care as prescribed by the lead nurse

- Develop understanding of caring for individuals with conditions for example dementia,mental illness, learningdisabilities

- Develop skillsin relation to coaching/teaching individuals/carers/other staff

- Assist with the implementation and monitoring of clinical standardsand outcomes

- Develop a working knowledge of other providersresources and referral systemsto ensure individuals needs are met, within parameters of practice

- Engage in reflective practice including management of self and reflection on own reactions, askingquestions and reflecting on answers given

- Demonstrate good understanding of principles of consent and ensurevalid consent is obtained prior to undertaking nursing and care procedures.

- Demonstrate good understanding of the Mental Health Act / Deprivation of Liberty Standards (DOLs)and applies principles to everydaypractice seeking advice / guidance from the Community Link Nurse or registered care professional as required

- Plan and manage competing demandsof job role, study, and placement activities.


Person Specification


Knowledge and Skills

* Clean Driving Licence and own transport
* Understanding of the scope of the role of the Nurse Associate in context of the team and the organisation, and how the role may contribute to service development.
* Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team.
* Insight into how to evaluate own strengths and development needs, seeking advice where appropriate.
* Evidence of time management skills and ability to prioritise.
* IT skills appropriate to the role
* Ability to communicate with members of the public and health and care providers.
* Courteous, respectful, and helpful at all times
* Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact
* Ability to work on own initiative, be well motivated and motivate others
* Ability to take part in reflective practice and clinical supervision activities
* Able to demonstrate resilience, managing personal emotions and stress.
* Knowledge of when to seek advice and refer to a registered care professional.
* Ability to engage and complete preceptorship
* Understanding of evidence based practice


Experience

* Previous experience of working within a health / care setting
* Understanding of the importance of the promotion of health and wellbeing (Making Every Contact Count)


Qualifications

* Registered Nursing Associate on the NMC register
* Care Certificate
* Educated to a minimum level 5 foundation degree specific to nursing associate practice.
* GCSE level 4-9 in Maths and English or equivalent
