Community Healthcare Support Worker

The closing date is 10 May 2026.

Do you want to make a real difference to patients? Are you passionate about supporting patients to remain in their own homes? Would you like to be part of a multi-disciplinary service working in the community and making a real difference to patients and their families?

We are actively recruiting Healthcare Community Support Workers to join our Urgent Response and Hospital at Home service, with vacancies across Eastleigh, Southampton, and Lymington.

The service runs from 8am to 8pm and we ask for a maximum of 2 weekends a month.

Our Urgent Response and Hospital at Home service provides rapid, short-term care in patients' own homes, helping to avoid unnecessary hospital admissions, support earlier discharge from hospital, and manage acute episodes safely in the community.

You will work as part of a friendly, experienced multidisciplinary team including nurses, therapists, paramedics, and doctors.

This is an excellent opportunity if you are:

* An experienced HCA, support worker, or carer
* Looking to move into an NHS community role
* Seeking a role with variety, autonomy and opportunities for learning and development

Please indicate your preferred location(s) when applying. Please note you will need a current driving licence and access to a vehicle for this role.

Main duties of the job

As a Community Support Worker, you will:

* Deliver delegated clinical, rehabilitation, and personal care in patients' homes
* Undertake observations and support assessments
* Support people with frailty, long‑term conditions, mobility needs, and end‑of‑life care
* Work as a lone worker, with robust clinical support
* Accurately record care using electronic systems

What We're Looking For

* NVQ Level 3 (or equivalent experience) or strong care experience
* Experience in a care, hospital, or community setting
* Ability to work independently and manage a varied workload
* Compassionate, reliable, and good communication skills

Why This Role?

* Opportunity to work across a variety of areas
* Full induction and supported competency development
* Daily clinical support – you are never working alone
* Meaningful patient contact – see the difference you make

If you have the right values, we will support you with the skills.

About us

Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides joined-up mental and physical healthcare for around two million people across our communities. With over 13,000 staff working in the community and local hospitals, we deliver care at every stage of life, helping people live their best and healthiest lives.

Our mental health services include community-based support and early intervention in psychosis (EIP) for both adults and young people, alongside a network of specialist inpatient wards covering forensic, learning disability, eating disorder and older person's care.

We deliver extensive physical health services too, from urgent community response teams helping frail and older patients remain safely at home, to hospitals at home teams providing acute‑level care in familiar surroundings. Our neurological services offer rehabilitation and treatment for conditions including Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.

Everything we do is underpinned by our CARE values of compassion, accountability, respect and excellence.
