Stevenage South PCN e are seeking a skilled advanced practitioner to join our supportive team and deliver high-quality, safe, patient-centred care. Based in Stevenage, Hertfordshire, this position is offered on a part-time basis for 16-22.5 hours/ 2-3 days per week (exact days to be agreed with the right applicant). This is a contract position, fixed-term until 31st March 2026 but has the potential for extension. Main responsibilities will include; Conduct thorough clinical assessments and formulate management plans for patients with minor illnesses. Assess, diagnose, and manage patients presenting with a variety of minor illnesses across primary and urgent care settings. Prescribe independently, providing efficient and effective treatment, reducing waiting times and improving patient outcomes. Work collaboratively with GPs, nurses, pharmacists, and other team members to deliver seamless care. Provide health promotion advice and support patient self-care. Maintain accurate clinical records and comply with all governance and regulatory standards. Please see attached additional information for further details. About the Candidate To be considered for this position, you will have the following; Registered healthcare professional (Nurse, Pharmacist, Paramedic, or other allied health professional) with a valid Independent Prescribing qualification. Experience and training in minor illness assessment and management. Strong clinical reasoning skills and sound knowledge of current prescribing guidelines (e.g., NICE). Excellent communication and interpersonal skills, with the ability to work well within a team and support fellow colleagues (clinical and non-clinical) Commitment to ongoing professional development and high-quality patient care.
