Job overview

We have a secondment opportunity for a nurse with experience of caring for people with MND to join our established Motor Neurone Disease multidisciplinary teamfor approximately 12 months. This is a very rewarding, privileged role which encompasses every aspect of the patient journey. You will be an integral part of the MND team who strive to ensure a coordinated and multi-professional approach to diagnosis, timely symptom management, interventions, and palliative care provision for all patients diagnosed with or suspected to have Motor Neurone Disease.

You will need to have excellent communication and organisational skills as well as the ability to work flexibly, autonomously and as a core member of the MDT.
Your role will involve assessing people living with MND from diagnosis throughout the disease trajectory. You must have the ability to work independently both in a hospital and community setting and to liaise with GPs, neurologists and other members of the MDT. You will need to be able to drive to patient’s homes and clinics including Lancashire and South Cumbria.

Main duties of the job

You will manage your own caseload and clinics, monitoring providing a systematic and holistic approach to the management of people with MND. You will provide education and support around diagnosis and symptom management, coordinating care and making referrals as appropriate. You will work 22.5 hours per week supporting the team to cover Monday to Friday between 8am-5pm.

Detailed job description and main responsibilities

The post holder will provide Clinical Nurse Specialist Support to the MND Care and Research Team, taking an active role in the delivery of high standards of care.

Contributing to the delivery of specialist services for people with MND, provide clinical advice, support to the organisation that leads and supports a reduction in health equalities and increase quality of life for patients with MND.

Undertaking of nurse led assessment and treatment clinics.

Contribute towards implementation towards national and local guidance and frameworks.

Qualifications

Essential criteria

1. RGN
2. Degree in health-related subject

Desirable criteria

3. Mentorship qualification
4. Clinical Examination Qualification
5. Diploma in Nursing

Knowledge

Essential criteria

6. Experience in and knowledge of Motor Neurone Disease
7. Experience of research in Motor Neurone Disease
8. Experience performing respiratory assessment in MND and use of monitoring equipment involved
9. Experience of ECAS assessments and onward referrals
10. Experience with Audits and service improvement
11. ACP experience including PPC documentation and support for ADRT, LPA and DNAR, including signposting to other professionals

Desirable criteria

12. Experience in developing guidelines or protocols
13. Experience in managing complex patients
14. Regional network, MDT and contact experience
