Integrated Care Clinician (Paramedic), Deeside Community Hospital
The post holder will undertake telephone triage assessments of patients in an efficient way.
They will advise the most appropriate intervention to meet the needs of the patient using best clinical practice, adhering to Trust Policy, protocols and guidelines.
Will work in an autonomous capacity, assessing and advising callers within agreed guidelines.

Responsibilities
The role involves:
* Telephone triage assessments and advising callers.
* Providing appropriate clinical interventions in line with best practice and Trust policy.
* Working autonomously within agreed guidelines and protocols.
* Supporting patients with a wide range of conditions from minor to complex and urgent.
* Collaborating with a multi‑disciplinary team including GPs, pharmacists and mental health practitioners.
* Developing remote clinical decision‑making skills.

Person Specification
Qualifications and/or Knowledge & Experience
* NMC Registered Nurse (adult or child), with relevant clinical practice as a nurse in front line, acute or chronic care, or telephone‑based triage, or HCPC Registered Paramedic with demonstrable post‑registration experience as a Paramedic in front‑line services.
* Evidence of undertaking regular update training and continuous professional development as required by the regulating body.

* Flexible approach to work – expect to work a rotating shift system including nights and weekends (unsocial hours).

Salary: £40,559 to £48,841 per annum (pro rata if part time).
