Practice Nurse – Fulwell Medical Centre
Location: Ebdon Lane, Sunderland, SR6 8DZ, United Kingdom
Working pattern: Part‑time, 30–37.5 hours per week (based on preference)
Contract: Permanent
We are excited to offer an excellent opportunity for an experienced General Practice Nurse to join our friendly and supportive clinical team. We provide high‑quality care to 9,200 patients and offer a teaching environment for medical students and GP registrars.
Key Responsibilities
* Support the Infection Prevention Control (IPC) lead with audits, training and compliance evidence.
* Publish the IPC annual statement alongside the Quality & Service Improvement Lead.
* Develop, implement and embed care management programmes for frail and housebound patients.
* Assess patient needs and provide evidence‑based healthcare.
* Deliver health promotion and wellbeing programmes.
* Implement and evaluate treatment plans for chronic disease patients.
* Identify and support patients at risk of developing long‑term conditions.
* Provide routine nursing care and acute‑chronic condition management according to NICE and NSF.
* Review patient medication for compliance and provide guidance on prescriptions and over‑the‑counter medicines.
* Coordinate travel medicine services and manage pathology requests and results.
* Oversee chronic disease clinics, referring as required.
* Maintain accurate clinical records and use SNOMED CT coding.
* Maintain chronic disease registers and run wellness clinics.
* Support GPs with minor surgery when needed.
* Prioritise health issues and intervene appropriately.
* Assist in clinical emergencies.
* Recognise, assess and refer patients with mental health needs.
* Implement vaccination programmes for adults and children.
* Support patients in using their medicines within scope of practice.
Secondary Responsibilities
* Develop and review audit protocols using the audit cycle.
* Review and implement nursing protocols with partners.
* Support nursing team through guidance, appraisal and training needs analysis.
* Participate in local initiatives, shared learning and quality improvement.
* Contribute to the review of near‑miss events and root‑cause analysis.
* Lead change planning and implementation within the nursing team.
* Review and implement practice policies and protocols in line with legislation.
Person Specification – Clinical Knowledge and Skills
* Experience with practice nurse skills: wound care, ECG, venepuncture, immunisations, new patient medicals and women’s health (cervical cytology, contraception).
* Request and interpret pathology tests.
* Evidence‑based practice and best‑practice promotion.
* Independent working within scope and appropriate referrals.
* Proficient IT skills for accurate clinical documentation.
* Effective communication with patients via telephone, email, face‑to‑face.
* Knowledge of safeguarding for adults and children.
* Understanding of GP surgery provision, QOF and enhanced services.
Qualifications
* Registered Nurse (Nursing and Midwifery Council).
* Evidence of practice level as per Primary Care and General Practice Nursing Career Framework.
* Mentor or teaching qualification (desirable).
* Family planning qualification (desirable).
Experience
* Broad general practice experience, including as a practice or community nurse.
* Chronic disease management.
* Autonomous working.
* Infection prevention and control measures.
* Managing elderly and housebound patients.
* Quality initiatives benchmarking (desirable).
* Knowledge of NHS landscape, PCNs, commissioners (desirable).
