Job Overview
The Urgent Community Response Team is a multi‑disciplinary team that responds to physical health crises in Havering, Barking, Dagenham and Redbridge.
In this role you will deliver high‑quality patient care by providing clinical assessment in the patient's own home to avoid hospital admission where not necessary.
Responsibilities
* Provide clinical assessment in patients' homes to avoid unnecessary hospital admissions.
* Thrive in a busy, demanding environment and cope effectively in crisis situations.
* Use strong clinical reasoning and communication skills to ensure patients are holistically assessed in a timely manner.
* Visit patients who may have fallen and be on the floor.
* Demonstrate professional curiosity and a commitment to high standards.
* Manage own workload, prioritise tasks, and meet deadlines.
* Work independently and as part of a team across multiple sites.
* Apply excellent communication and organisational skills.
Employment Terms
1‑year fixed‑term contract. The post is subject to a probationary period (internal applicants exempt unless currently subject to probation or a bank member).
Salary and Benefits
This post attracts a High Cost Area Supplement of 15% of the basic salary, with a minimum of £4,714 to a maximum of £5,941 p.a. pro‑rata for part‑time.
