Would you like to play a part in the Hospital at home service in the beautiful location of Hambleton and Richmondshire.
We have a rare opportunity for a 9-month maternity leave cover to lead and oversee clinical pharmacy input to the Urgent Community Response (UCR) and virtual ward team to deliver care to the virtual ward, community beds and Urgent Care Response teams.
You will contribute to patient care both within the unit base setting, and via patient home visits where appropriate.
Lead on medication provision and delivery across organisational boundaries.
Complete Medication reviews,medicinesreconciliationandpharmaceuticalcareplans for patients admitted to the virtual ward.
Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.
Main duties of the job
You will work as an independent prescriber, optimising medicines and promoting appropriate, evidence-based, and cost-effective prescribing and deprescribing as part of the wider hospital at home team.
You will be using your clinical skills and knowledge in investigations, analysing, and implementing actions for medication related incidents within the virtual ward.
The provision of specialist clinical pharmacy knowledge to a range of health and social care professionals on the safe prescribing, handling, and administration of medicines.

Qualifications & training
Essential criteria
1. Qualified to a degree level (MPharm or equivalent)
2. Registration with the General Pharmaceutical council or Great Britain as a practising pharmacist.
3. Post graduate Diploma in clinical pharmacy, or equivalent.
4. Non-medical prescriber qualification
Desirable criteria
5. RPS Faculty membership.
Knowledge and skills
Essential criteria
6. Professional knowledge acquired through vocational degree in pharmacy
7. Signficant pre-registration training and experience
8. Significant post-qualification experience in a relevant role, practising as a pharmacist in hospital or primary care
9. Good all-round clinical knowledge and practical clinical skills
10. Experience of supervising staff and leading a team
11. Able to work autonomously and within a team

13. Worked as a clinical pharmacist at a high level
14. Creating, developing, and reviewing protocol and/or guidelines
