Job Overview
We are seeking a qualified District Nurse or a recent graduate of the District Nurse Specialist Practice Qualification to join the Community Nursing Networks in North Lincolnshire.
Role
As a District Nurse you will manage a caseload of complex patients and support team and student learning in a forward‑thinking, integrated health and social care environment.
Responsibilities
* Manage complex patient caseloads and contribute to team management.
* Update and populate rosters, handle daily sickness coverage, and support the wider workforce.
* Complete projects and other duties required by the role.
* Collaborate closely with GPs and other multidisciplinary team members to meet end‑of‑life care and long‑term conditions agenda.
Qualifications & Experience
* Fully qualified District Nurse or holder of the District Nurse Specialist Practice Qualification.
* Band 6 level, with sound understanding of current nursing and health agenda.
* Excellent communication, leadership and decision‑making skills.
* Ability to work autonomously and promote a positive learning environment for team members and students.
Preceptorship
This post offers a 6‑month preceptorship period for newly qualified District Nurses to develop caseload management skills.
