Cambridgeshire and Peterborough NHS Foundation Trust (CPFT) invite applicants for a Mental Health Liaison Practitioner post. We are recruiting for a part time vacancy in the Fenland area.

CPFT was one of the national exemplar pilot sites invited to roll out their model for radical change in the delivery of community mental health services in. The Adult Community Mental Health Transformation seeks to offer place based local care, eliminate current gaps between services and join up mental health care delivery.

The Primary Care Mental Health Service sits at the heart of this transformation in Cambridgeshire and Peterborough as it is organised around local PCNs/Neighbourhoods, can refer people directly into new services in CPFT which are filling the gaps between IAPT and secondary care. Its Mental Health Liaison Practitioners (MHLPs) work closely with primary care, statutory and non-statutory colleagues to ensure that care is joined up.

Main duties of the job

Clinical Leadership and Triage

1. Act as the senior clinician within the PCN, triaging all GP referrals to ensure appropriate allocation within the team.
2. Undertake assessments for individuals with the most complex mental health needs.
3. Work closely with consultants and advanced nurse prescribers to facilitate access to multidisciplinary support .

Collaborative Working

4. Attend virtual clinics with GPs and monthly meetings with the PCN mental health lead.
5. Liaise with statutory and non-statutory services to coordinate care for people with complex mental health needs.
6. Build strong relationships with local community groups to promote engagement and equality of access, particularly for ethnic minority populations.

Community Engagement and System Integration

7. Participate in neighbourhood-level initiatives and maintain close links with Integrated Care System (ICS) project and programme leads.
8. Represent the service at Community Safety Partnership Boards and other local forums to support joined-up care and community safety.
