Re-ablement is central to the service provided by Kirklees Council's Short Term and Urgent Support Team.

Our ethos is that every resident of Kirklees should have the access to support which explores opportunities for re-ablement potential or minimizes reliance on longer term services and support .

Ensuring minimum CQC care standards are exceeded, the short term and urgent support teams provide support to people enabling them to remain or return home following an acute episode or a sudden deterioration in their health. Your role will be to case manage and work with individuals to reach their full potential and to remain as independent as possible.

This role is within the Out of Hours Team and is crucial in supporting Home Support Co-ordinators and Home Support Workers to ensure individuals receive the necessary care to remain at home, promoting their independence and recovery between the hours of 7am and 11pm.

As a Locality Manager in the Out of Hours & Mobile Response Team, you will be part of a team of professionals, including a Team Manager, Locality Managers and Social Care Responders. You will oversee and be responsible for activities while on shift for the broader service team, leading Home Support Co-ordinators and Home Support Workers, including the management of crisis situations. Occasionally, you will also deputize for the Team Manager.

Key Responsibilities

Provide high-quality support services for service users.
Ensure timely responses to urgent requests for support.
Successfully re-able service users to regain independence, reducing the need for long-term support.
Exceed minimum CQC care standards.
Equip staff with the appropriate skills and resources to deliver services efficiently and effectively.
Promote and foster effective working relationships with partner agencies to ensure the highest standards in service delivery and improvements.
Maintain positive working relationships and effective communication.
Develop support packages that meet the individual needs of service users, utilizing creative approaches and technology.
Deliver services within localities efficiently and effectively, contributing to benefits realization and the Council's green agenda.
Deploy staff appropriately, ensuring they have the relevant range of skills to perform their duties over a dispersed geographical area.
Safeguard vulnerable adults.
Manage a delegated budget effectively.

Requirements

Strong leadership skills.
IT literate
A full and valid driving license, as travel is essential.
An enhanced Disclosure and Barring Service check.
