Position Overview
Hospital Discharge Social Work Team has a new opening for a Social Worker at Hairmyers Hospital, East Kilbride. This is an exciting opportunity for experienced Social Work Practitioners who seek to further develop their practice within the University Health and Social Care Partnership.
Role Details
Location: Hairmyers Hospital, East Kilbride
Hours: 35hrs per week, permanent
Salary: £47,611 - £48,633
Responsibilities
* Work with adults and older people requiring support planning to discharge from hospital.
* Collaborate with supported people, families, carers, and the wider multidisciplinary team in both the acute hospital setting and the community setting for discharge planning.
* Gather information, complete assessments and demonstrate competence in risk assessment and management with vulnerable adults.
* Provide adult support and protection and adults with incapacity legislative duties, including attending MDTs and facilitating discharge referrals.
Qualifications & Experience
Experienced Social Work Practitioner with competence in assessing risk related to working with vulnerable adults and knowledge of adult support and protection legislation.
