3 months contract with a Local Authority
Job Summary:
• The Day Opportunities Facilitator will support individuals with a range of personal, emotional, and practical needs in community hubs, specialist services, and wider community settings.
• The role involves delivering structured day opportunities and activities, contributing to care planning, and supporting individuals to promote independence, wellbeing, and personal outcomes.
• Personal care and support may be required, and work will be carried out both in group settings and on a 1:1 basis.
Key Duties/Accountabilities (Sample):
• Provide day-to-day assistance in response to individuals' personal, emotional, and physical needs as outlined in care plans, always maintaining dignity and respect.
• Deliver a variety of planned activities within the hubs or in the community in line with care plans and the service’s quality assurance framework.
• Positively support individuals within a risk management framework and escalate any concerns appropriately.
• Contribute to the review, development and monitoring of care plans and risk assessments through consultation with service users, carers, and other professionals.
• Promote and maintain independence, involvement, and empowerment of individuals accessing day opportunities.
• Assist with personal care, medication administration (as trained), and use of equipment as specified in care plans.
• Communicate effectively with individuals, carers, families, and external agencies.
• Participate in staff meetings, training, and contribute to service improvement initiatives.
• Maintain accurate records, adhere to confidentiality policies, and comply with health and safety legislation.
• Support transport needs, including escorting service users and driving vehicles as required.
• Respond to emergency or crisis situations calmly and appropriately, reporting incidents promptly.
• Provide cover for colleagues and work flexibly to meet service demands.
Skills/Experience:
• Previous experience in delivering care or support services within community or day opportunity settings.
• Understanding of personal outcomes and enabling individuals to live the life they want.
• Ability to provide respectful and dignified personal care.
• Experience supporting individuals with complex needs and/or behaviours of concern.
• Strong communication skills – verbal and written – with service users, families, carers, and professionals.
• Able to work both independently and as part of a team.
• Proficient in the use of basic ICT systems.
• Good organisational skills with the ability to prioritise workload effectively.
• Commitment to safeguarding and creating a safe environment for all.
Additional Information:
• A relevant care qualification or willingness to work towards Level 2 Diploma in Adult Care and the Care Certificate.
• Part-time role: 30 Hr/week.
