An exciting opportunity has become available for a full time band 5 Community Children’s Nurse to work within the Bexley and Greenwich Community Children’s Nursing Team and integrated Hospital at Home Team.
The team provides a seven day service from 08.00 to 20:30, with a 24 hour on call provision for children & young people who require end-of-life care at home.
The team of specialist and generic children’s community nurses provide skilled, comprehensive nursing care to children and families with nursing needs in their own homes or community settings in the boroughs of Bexley and Greenwich.
Responsibilities
* Provide a high standard of evidence‑based nursing care for children and young people with acute and long‑term conditions, children with disabilities and complex conditions, including those requiring palliative and end‑of‑life care.
* Work in partnership with other community‑based statutory and voluntary organisations to deliver seamless services for children and their families.
* Travel between sites and patients' homes across the boroughs of Bexley and Greenwich.
* Share a caseload with a senior member of the nursing team following completion of the competency framework and work without direct supervision.
* Deliver a seven‑day service from 08.00 to 20:30 and provide 24‑hour on‑call provision for end‑of‑life care at home.
