Job Description Salary scale: £13.67 per hour Contract: Fixed term until 31st March 2026 Hours: 16 hours per week Base: Kenburgh House, 28 Manor Row, Bradford, BD1 4QU with requirement to travel within (Bradford, Airedale, Wharfedale or Craven). Accredited employer with Mindful Employer, Disability Confident and Real Living Wage. ​ Role Purpose: Bradford District and Craven Mind is looking for a dedicated person committed to delivering high-quality mental health care and support for the SMI-LE service. The role involves providing one-to-one support for adults experiencing moderate to severe mental health challenges. Travel within Bradford, Airedale, Wharfedale, or Craven is required to meet with service members and offer support. ​ Support will focus on areas affecting well-being, including employment, welfare and benefits, social housing, money management, social isolation, and bereavement. A good understanding of local support services across the district is essential, as the role involves signposting and completing onward referrals for service members to relevant services for additional support. ​ Support is flexible and may take place in the service member’s home, in the community, or at the local Community Mental Health Team base. It can be delivered face-to-face, by telephone, or via video call, depending on the individual’s preference. The approach ensures that the service member remains at the center of all decision-making and support planning. ​ The role also includes administrative responsibilities, such as supporting with triaging, liaising with professionals, and following up on identified risks to ensure appropriate actions and support are in place. Key duties ​ Complete 1- 1 support for adults facing moderate to severe mental health challenges face-to-face and or over the telephone Provide support that meets the service member needs. This support will be around client well-being, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement. Have a good understanding of support services across the district and signpost/complete onward referrals for service members to relevant services for additional support Work in partnership with other services across Bradford District and Craven to ensure the best outcome for service members Record information electronically and maintain up to date service member records Take responsibility for ensuring Safeguarding, Information Governance and Equality & Diversity requirements, and all other Company policies are complied with and seek to improve these where possible. Work in alignment with the policies, aims, objectives and values of Bradford District and Craven Mind Perform any other duties from time to time that may be reasonably required to ensure the smooth running of Mind in Bradford. General duties Foster and maintain strong relationships with stakeholders, both internal and external, representing the organisation and facilitating effective communication channels Person specification Knowledge: Essential Understanding of the key issues within mental health, and the potential challenges that people may face Knowledge of other relevant health & social care organisations across the Bradford District & Craven Knowledge of risk management and health and safety issues Understanding of safeguarding and good practice Understanding of confidentiality and data protection Experience Essential Experienced and confident in managing challenging situations, and recognising and responding to safeguarding concerns (this can include voluntary experience) Experience of working with children, young people or adults in a health, social care or any other relevant setting (this can include voluntary experience) Experience of supporting people in a clear, meaningful and non-judgemental way either via telephone or in person (this can include voluntary experience) Skills and abilities Essential Ability to gather relevant information to support and assess risks Excellent proven communication and organisational skills, including an ability to encourage and engage people in ways to make positive changes in their lives.
