Job overview

1. To manage own caseload of Children with a Learning Disability, which will be identified by Senior Community Nurse for Children with a Learning Disability.

2. To plan care for Children with a learning disability and be a support to them and their families according to the care pathways.

3. To be a competent, autonomous community practitioner, with support from senior members of the Learning Disability Team.

4. To work as a member of Children and Young Peoples service to promote a seamless provision for children.

Main duties of the job

1.To provide care in partnership with parents/carers/children using interventions appropriate to the assessed need as per the CNCLD Standard Operating Procedures.

2. To evaluate the outcome of interventions using a process of regular review in collaboration with the child, family, CNCLD and other professionals involved.

3. To advise on the promotion of health and the prevention of ill health.

4. Participate in Safeguarding Children processes as required, sharing information as necessary and completing reports within timescales.

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria

10. Registered Nurse/Awaiting NMC PIN

Desirable criteria

11. Evidence of further continued professional development
12. Post qualification experience in relevant speciality
13. Hold / be working towards an NMC approved mentoring qualification

EXPERIENCE

Essential criteria

14. Awareness of current professional nursing issues
15. Demonstrates understanding of the importance of audit/quality

Desirable criteria

16. Post registration experience working with Children with Additional needs

SKILLS, ABILITIES & KNOWLEDGE

Essential criteria

17. Good verbal and written communication skills and ability to negotiate
18. Ability to manage and prioritise own caseload with supervision
19. Ability to solve problems and use initiative
20. Ability to develop clinical skills
21. Willingness to undergo training (e.g. medications)
22. Basic IT skills
23. Knowledge of the NMC code of conduct
24. Ability to maintain a safe clinical environment conducive to patient care Adaptable to change
25. Ability to work rotating shifts
26. Diplomatic approach with professionalism
27. Flexible and approachable

Desirable criteria

28. Willingness to develop teaching skills
29. Awareness of the NHSE guidance of clinical governance in order to continuously improve quality of care e.g. quality, safety and ward audit standards
30. Knowledge of care planning, preceptorship, reflection and clinical supervision

SPECIAL CIRCUMSTANCES

Desirable criteria

31. Able to travel to other sites and off site as necessary
32. Maybe required to work in other departments if requested to do so
