Overview
This is an excellent opportunity to join one of the leading haemoglobinolopathy services in the country at Imperial College Healthcare NHS Trust. The post will form part of an established team of Clinical Nurse Specialists located mainly at Hammersmith site supporting adult and TYA (teen and young adults) services trust wide and Northwest London hospitals.
You will need to have haematology nursing experience, and ideally some of this should include working in the speciality as a junior sister or in a specialist nursing role directly involved in the management of patients with haemoglobin disorders. The post requires exceptional organisational skills, managing a patient caseload, working autonomously and as part of the wider MDT. Successful candidates must be highly motivated and forward thinking nurses who are interested in positively influencing clinical pathways for patient with Haemoglobinopathies.

Responsibilities
* To work as autonomous practitioners providing specialist nursing advice, care and support, facilitating high quality, individualised care to patients and those important to them.
* To provide support to inpatients and outpatients in collaboration with the multi-disciplinary team and in line with local and national targets and initiatives.
* Provision of specialist, high quality, expert nursing care and support for adult patients and their carers with a diagnosis of an inherited haemoglobinopathies being assessed, co-ordinated or cared for within ICHNT and in liaison with the multidisciplinary team and other health agencies.
* To perform an effective role within the MDT linking with relevant healthcare teams in order to increase knowledge and facilitate the care of patients in the community or in their local healthcare setting.
* To act as a specialist resource for patients and the clinical team by providing comprehensive advice, information, teaching and training both within ICHT and externally.
