Social Prescribing Link Worker (Complex Mental Health Focus)

Were looking for amotivated and compassionate Social Prescribing Link Worker to join our GreaterMiddlesbrough Social Prescribing Team, with a dedicated focus on supportingpeople experiencing complex mental health needs.

Working across GP practices within the Greater MiddlesbroughPrimary Care Network, youll provide personalised, holistic, and traumainformedsupport to individuals who may face barriers to accessing services orexperience health inequalities.


Main duties of the job

Work onetoone with people to explore their strengths, goals and the challenges they face.

Connect individuals with community groups, activities, and services that enhance their wellbeing, confidence and social connections.

Support people to develop coping strategies, improve emotional resilience and navigate difficult circumstances.

Work in partnership with primary care and community organisations to ensure the best outcomes for those you support.

Help identify gaps in local provision and contribute to the shaping of future services.

Youll need to be caring, enthusiastic and highly organised, with experience of working in traumainformed ways and supporting people with complex or multiple needs. Strong communication skills, the ability to work independently, and a commitment to empowering individuals are essential.

Job responsibilities

Main Duties

Development

1. Work within the Greater Middlesbrough PCN to promote the role of Social Prescribing for patients.
2. Work in partnership with primary care staff to increase understanding of patients needs and how they can be met through social prescribing and existing community provision.
3. Work in partnership with local community provision to include and support individuals referred into the service.
4. Support the team to identify gaps in services and support available to individuals, and feed this into service development and stakeholder discussions.

Delivery

1. Respond to referrals from GPs and practice staff and engage individuals in the service.
2. Deliver a service that is flexible, person-centred and trauma informed.
3. Support patients through the service pathway, including:
o Triage, assessment and delivery of time specified, goal focused support
o Working with each individual to identify their strengths and goals
o Co‑produce a plan with each individual to work towards their goals
o Introducing and connecting individuals to organisations, groups, activities and services relevant to them
o Co‑produce a safety assessment and plan to manage concerns around risk and vulnerability
4. Deliver a range of interventions including:
o Supporting individuals to attend community groups and activities
o Providing emotional support and confidence‑building
o Helping individuals to develop coping strategies
o Supporting individuals to maintain relationships with friends and family
5. Gather and record progress and outcomes with individuals, including:
o Using an agreed outcome measurement tool
o Using electronic database/case management systems (including GP practice systems -System1/EMIS)
6. Capture, understand and learn from feedback provided by individuals using the service.
7. Attend GP practice meetings, where required, and provide relevant information and feedback.
8. Manage a caseload of patients, ensuring referrers understand capacity and possible waiting times.

Other Responsibilities

1. Work within the values and policies of Teesside Mind
2. Proactively participate in in line management supervision, caseload management supervision and peer support sessions
3. Ensure confidentiality is upheld, in line with legislation and organisational policies
4. Ensure health and safety of self and others
5. Attend training and events as required
6. Undertake other tasks commensurate with the position as required.


Person Specification


Qualifications

* Level 3 health and social care qualification or equivalent


Experience

* Working within trauma-informed approaches
* Engaging and working with those who may experience barriers to accessing services and who have complex health and social issues
* Working collaboratively with individuals in goal focused ways
* Close partnership working with other organisations / services for the benefit of the individual
* Working independently in the community (or in a relevant setting)
* Using outcome measurement tools with individuals (desirable)
* Working closely with or within primary care settings (desirable)
* Facilitating peer led support groups (desirable)
