Job Summary
We know that service users recover quickly at home, and you will be an integral component for ensuring that this is done safely and specifically as an alternative to hospital admission, working alongside the wider MDT.
Responsibilities
* Provide a clinical service that is service‑user focused, accessible and responsive 24/7.
* Triage all mental health referrals to determine priority for response and appropriate clinical pathway.
* Provide and deliver highly skilled nursing care, including a full mental health assessment, care planning and delivery, biopsychosocial interventions, clinical risk assessment and management, and promotion of recovery and inclusion.
* Recommend and refer clients, where treatment is indicated, to the most appropriate team or service.
* Undertake gate‑keeping assessments for inpatient and crisis house admissions.
* Collaborate with the crisis house to lead on care for admissions as a service.
* Facilitate early discharge from inpatient wards where clinically indicated.
* Deliver home‑based treatment interventions for all clients, ensuring best practice is implemented.
* Participate in Mental Health Act assessments where appropriate to consider alternatives to formal hospital admission.
* Provide clinical leadership and supervision for junior staff.

Person Specification
Essential Experience
* Demonstrable post‑registration experience as a Registered Mental Health Nurse.
* Professional experience in mental health.
* Experience of MDT working.
* Skills in coordinating and planning inter‑agency approaches to mental health care.
* Experience of assessing and managing risk.
Essential Qualifications
* Registered Mental Health Nurse with current professional registration with the NMC.
* Evidence of continuing professional development.
* ENB 998 or other NMC‑approved mentor qualification.
* Key skills in literacy, numeracy and IT Level 2 or equivalent.
Desirable
* Risk Assessment and Management training.
* Suicide Prevention training.
* Supervision training.
